摘要
目的探讨呼吸反馈+个性化堵管技术在脑卒中后气管切开患者中的应用效果。方法选取2019年1~12月广西壮族自治区柳州市人民医院收治的40例脑卒中后气管切开患者,按治疗方式不同将其分为对照组和观察组,每组各20例。对照组按传统呼吸及机械辅助排痰+常规拔管技术,观察组按呼吸反馈+个性化堵管技术。比较两组拔管后的呼吸困难、血氧饱和度(SpO2)、气管套管带管时间、肺部功能状况。结果观察组的治疗总有效率为95.00%,高于对照的50.00%,差异有统计学意义(P<0.05);治疗前两组的肺部功能状况比较,差异无统计学意义(P>0.05);观察组治疗后的第1秒用力呼气容积(FEV1)、FEV1占用力肺活量(FVC)的百分比(FEV1/FVC)高于对照组,差异均有统计学意义(P<0.05);治疗前两组的最大吸气量、最大呼吸量比较,差异无统计学意义(P>0.05),观察组治疗后的最大吸气量、最大呼吸量高于对照组,差异均有统计学意义(P<0.05);观察组的肺部感染未发生率为95.00%,高于对照组的65.00%,差异有统计学意义(P<0.05);观察组的呼吸困难发生率、SpO2<90%发生率低于对照组,差异均有统计学意义(P<0.05)。结论脑卒中后气管切开患者采用呼吸反馈+个性化堵管技术对肺康复效果显著,可有效缓解肺部功能,缩短气管切开患者的带管时间,减少拔管对患者呼吸的影响,值得临床应用。
Objective To explore the application effect of individualized blocking of respiratory feedback in patients with tracheotomy after stroke.Methods A total of 40 patients with tracheotomy after stroke in Liuzhou People′s Hospital,Guangxi Zhuang Autonomous Region from January to December 2019 were selected.According to the different treatment,they were divided into the control group and the observation group,20 cases in each group.The control group used traditional inhalation and mechanical assistance for expectoration+conventional extubation technique,the observation group used personalized blocking technique of respiratory feedback+individualized pipe plugging technology.After extubation,dyspnea,oxygen saturation(SpO2),tracheal cannulation time and pulmonary function were compared between the two groups.Results The total effective rate in the observation group was 95.00%,higher than that of the control group of 50.00%,the difference was statistically significant(P<0.05).The pulmonary function of the two groups before treatment was compared,there was no significant difference(P>0.05).The forced expiratory volume(FEV1)in the first second and the percentage of FEV1 in forced vital capacity(FVC)(FEV1/FVC)in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).There were no significant differences between the two groups before treatment in the maximal inspiratory volume and maximal inspiratory volume(P>0.05),the maximal inspiratory volume and maximal inspiratory volume in the observation group were higher than those in the control group(P<0.05).The non-incidence rate of pulmonary infection in the observation group was 95.00%,higher than that in the control group(65.00%),the difference was statistically significant(P<0.05).Incidence of dyspnea and the incidence of SPO2 were lower than those of control group,the differences were statistically significant(P<0.05).Conclusion Breathing feed back used in post-stroke patients with tracheotomy+individualized pipe plugging technology has a remarkable effect on lung rehabilitation,it can effectively relieve the function of the lungs,shorten the intubation time of tracheotomy patients,to reduce the effect of extubation on inhalation of patients,worthy of clinical application.
