摘要
目的:探讨不同妊娠期妇女甲状腺功能与碘营养水平的特点。方法:分析在医院产科门诊随访的93例妊娠妇女的尿碘及甲状腺功能情况,依据不同孕期分为孕早期(26例)、孕中期组(31例)以及孕晚期组(36例)。观察比较三组孕妇的促甲状腺素(TSH)、游离甲状腺素(FT4)、游离三碘甲状腺原氨酸(FT3)水平,尿碘(UIC)水平及经肌酐校正后的尿碘(UIC/UCr)水平,以及亚临床甲状腺功能减退症的发生率。结果:93例妊娠妇女尿碘中位数为114.6μg/L,处于轻度碘缺乏状态。三组孕妇UIC/UCr分别为152.45(99.36~229.85)μg/g、208.31(158.81~345.26)μg/g、220.59(160.77~307.74)μg/g。三组孕妇的FT4、TSH水平差异无统计学意义(P>0.05),在孕晚期FT3水平则较孕早期及孕中期下降(P<0.001)。亚临床甲状腺功能减退症发生率总体较低,其中孕早期组未见亚临床甲状腺功能减退症发生。结论:对于妊娠过程及妊娠结局而言,保持正常的甲状腺功能具有重要意义,本研究中合肥地区妊娠妇女总体甲状腺功能紊乱发生率不高,需加强妊娠期碘营养指标及甲状腺功能动态监测。
Objective:To study the characteristics of thyroid function and iodine nutrition level in different pregnant women.Methods:The urinary iodine and thyroid function of 93 pregnant women who were followed up in the obstetric clinic of the Hospital.The levels of TSH,FT4,FT3,urinary iodine,urinary iodine corrected by creatinine and the incidence of subclinical hypothyroidism were observed and compared among the three groups.Results:The MUI of 93 pregnant women was 114.6μg/L,which was in mild iodine deficiency state.The UIC/UCR of the three groups were 152.45(99.36-229.85)μg/g、208.31(158.81-345.26)μg/g、220.59(160.77-307.74)μg/g.There was no significant difference in FT4 and TSH levels among the three groups(P>0.05),but the level of FT3 in the third trimester was lower than that in the first and second trimester(P<0.001).The incidence of subclinical hypothyroidism was generally low,and no subclinical hypothyroidism was found in the early pregnancy group.Conclusion:For the pregnancy process and pregnancy outcome,it is important to maintain normal thyroid function.In this study,the overall incidence of thyroid dysfunction in pregnant women in Hefei area is at a low level.It is necessary to strengthen the iodine nutrition index and thyroid function dynamic monitoring during pregnancy.
