摘要
目的探讨哮喘儿童急性发作期外周血细胞因子白介素-10(IL-10)、白介素-12(IL-12)、肿瘤坏死因子-α(TNF-α)、转化生长因子-β(TGF-β)水平及影响因素。方法对2018年12月至2019年12月期间收治的78例哮喘急性发作期儿童和同期体检的45名健康儿童外周血清中IL-10、IL-12、TNF-α、TGF-β四种细胞因子水平进行分析和比较。结果哮喘儿童急性发作期外周血清中细胞因子IL-10、TGF-β浓度明显高于健康儿童(14.2±6.5,t=6.876,P=0.000;374.9±307.9,t=6.672,P=0.000);哮喘急性发作期儿童中临床重度及危重度患者外周血清中细胞因子IL-12、TNF-α较轻中度患者明显升高(10.5±4.5,t=3.698,P=0.033;15.4±6.6,t=4.675,P=0.000)。哮喘儿童急性发作期外周血清中四种细胞因子水平在嗜酸性粒细胞(EO)、免疫球蛋白E(IgE)及C反应蛋白(CRP)正常组和升高组中比较无明显差异(均P>0.05)。结论哮喘儿童急性发作期外周血存在细胞因子表达异常,细胞因子的异常表达与过敏、感染等炎症指标无明确的相关性,提示儿童哮喘可能与免疫异常有关。
Objective To explore the levels of cytokines IL-10,IL-12,TNF-α,TGF-β in peripheral blood in children with bronchial asthma during acute attack stage and its influencing factors.Methods The levels of IL-10,IL-12,TNF-α,TGF-β in peripheral blood of 78 children with bronchial asthma during acute attack stage who were admitted to the integrated Chinese and Western Medicine Hospital of Zhejiang Province from December 2018 to December 2019 and 45 heathy children who underwent physical examination in the same period were analyzed and compared.Results The levels ofIL-10,TGFβin peripheral serum in Children with bronchial asthma during acute atack stage were signifcantly higher than those in healthy children(14.2±6.5,t=6.876,P=0.000;374.9±307.9,t=6.672,P=0.000).And the levels ofIL-12 and TNF-a in peripheral serum in children who had severer clinical manifestations with bronchial asthma during acute attack stage were significantly higher than those in milder patients(10.5±4.5,t=3.698,P=0.033;15.4±6.6,t=4.675,P=0.000).The levels of four cytokines in peripheral serum in children with bronchial asthma during acute attack stage showed no significant diference between eosinophilic granulocyte,IgE and CRP in normal and elevated groups(P>0.05).Conclusion Abnormal expression of cytokines is found in peripheral blood in children with bronchial asthma during acute attack stage,bu there is no clear correlation between the abnormal expression of cytokines and indicators such as alergy and infection,suggesting that asthma in children may be related to immune abnormalties.
