摘要
目的探讨舒芬太尼和瑞芬太尼在ICU镇痛治疗中的药物经济学价值,为临床提供用药选择依据。方法将30例危重程度相当并且均需要呼吸机支持治疗的患者分为2组,A组(瑞芬太尼镇痛)15例,B组(舒芬太尼镇痛)15例。观察临床镇痛效果、对生命体征的影响以及不良反应,计算2组成本-效果比,同时进行敏感度分析,应用药物经济学进行评价。结果2组患者用药后30 min和1 h,HR、RR、MAP比较差异无统计学意义(P>0.05)。A组治疗总有效率为98.9%,B组总有效率为98.4%,2组镇痛评分比较差异无统计学意义(P>0.05)。A组和B组治疗成本分别为10583.0元、10796.5元,A组成本-效果为107007.08,B组为109720.53,敏感性分析与成本-效果分析结果一致。结论结合药物治疗学、药物安全性及药物经济学,瑞芬太尼可能是ICU重症患者镇痛治疗的一个较好选择。
Objective To evaluate pharmacoeconomic value of sufentanil and remifentanil in analgesic therapy in ICU.Methods Thirty critically ill patients requiring ventilator support were randomly divided into two groups(A group and B group).The patients in group A were given remifentanil for analgesia,and the patients in group B were given sufentanil for analgesia.Thr clinical effect and adverse reactions were observed and the cost-effectivness ratio of the two groups was calculated.At the same time,the sensitivity analysis and pharmacoeconomic evaluation were applied.Results There was no significant difference in HR,RR and MAP between the two groups 30 minutes and 1 hour after treatment(P>0.05).The total effective rate was 98.9%in group A and 98.4%in group B.In addition,there was no significant difference in analgesic score(P>0.05).The cost of treatment in group A and B was 10583.0 and 10796.5 yuan respectively.Meanwhile,the cost-effectiveness of the two groups was 107007.08 and 109720.53,which had statistical significance.Sensitivity analysis and cost-effectiveness analysis were consistent.Conclusion In consideration of pharmacotherapeutics,drug safety and pharmacoeconomics remifentanil may be a better choice in the analgesic therapy.
