摘要
目的探究女性甲功异常患者血清游离三碘甲状腺原氨酸(F T 3)、血清游离甲状腺素(F T 4)、促甲状腺激素(TSH)与性激素水平的相关性。方法回顾性分析2019年1月~2020年1月我院女性甲功异常患者63例临床资料,依据检查、诊断结果不同分组,将其中甲状腺功能亢进的31例患者归为甲亢组,将甲状腺功能减退的32例患者归为甲减组,另选取同期于我院检查显示无任何问题的30例健康者作为健康组。比较3组甲状腺功能水平和血清性激素水平,并分析FT3、FT4、TSH与性激素水平的相关性。结果甲亢组FT3、FT4高于健康组,TSH低于健康组,差异有统计学意义(P<0.05);甲减组FT3、FT4低于健康组,TSH高于健康组,差异有统计学意义(P<0.05);甲亢组、甲减组雌二醇(E2)高于健康组,差异有统计学意义(P<0.05);甲亢组、甲减组促卵泡激素(FSH)及促黄体生成素(LH)与健康组比较,差异无统计学意义(P>0.05);经Pearson分析,女性甲亢者FT3、FT4与E2水平均呈正相关(r>0,P<0.05),TSH与E2水平间呈负相关(r<0,P<0.05);女性甲减患者FT3、FT4与E2水平均呈负相关(r<0,P<0.05),TSH与E2水平呈正相关(r>0,P<0.05)。结论女性甲亢者FT3、FT4与E2水平均呈正相关,TSH与E2水平间呈负相关;甲减者FT3、FT4与E2水平均呈负相关,TSH与E2水平呈正相关。
Objective To explore the correlation between serum free triiodothyronine(FT3),serum free thyroxine(FT4),thyroid stimulating hormone(TSH)and sex hormone levels in female patients with abnormal thyroid function.Methods The clinical data of 63 female patients with abnormal thyroid function from January 2019 to January 2020 were retrospectively analyzed.According to different examination and diagnosis results,31 patients with hyperthyroidism were classified into hyperthyroidism group,32 patients with hypothyroidism were classified as hypothyroidism group,and 30 healthy people who had no problems on the examination in our hospital during the same period were selected as the healthy group.The thyroid function and serum sex hormone levels of the three groups were compared,and the correlation between FT3,FT4,TSH and sex hormone levels was analyzed.ResuIts FT3 and FT4 in hyperthyroidism group were higher than healthy group,TSH was lower than healthy group,the difference was statistically significant(P<0.05);FT3 and FT4 in hypothyroidism group were lower than healthy group,TSH was higher than healthy group,the difference was statistically significant(P<0.05);Estradiol(E2)in hyperthyroidism group and hypothyroidism group was higher than that in healthy group,the difference was statistically significant(P<0.05);Compared the follicle stimulating hormone(FSH)and luteinizing hormone(LH)in the hyperthyroidism group and hypothyroidism group with the healthy group,the difference was not statistically significant(P>0.05);according to Pearson analysis,there was a positive correlation between FT3,FT4 and E2 levels in female hyperthyroidism(r>0,P<0.05),there was a negative correlation between TSH and E2 levels(r<0,P<0.05);female patients with hypothyroidism had a negative correlation between FT3,FT4 and E2 levels(r<0,P<0.05),and there was a positive correlation between TSH and E2 levels(r>0,P<0.05).ConcIusion The FT3,FT4 and E2 levels of female hyperthyroidism were positively correlated,and the TSH and E2 levels were negatively correlated;the FT3,FT4 and E2 levels of hypothyroidism were all negatively correlated,and TSH and E2 levels were positively correlated.
