摘要
目的:了解中成药治疗慢性肾脏病(CKD)的现状和优势特色,为《中成药治疗慢性肾脏病临床应用指南》的临床问题构建提供依据。方法:采用专家问卷调查的方式,于2018年04月03日~17日期间,在中国中医药协会肾病中药发展研究专业委员会全委中选取有三级甲等医院临床工作经验10年以上、副主任医师以上职称的专家共49名进行调查。结果:发放问卷49份,回收46份,回收率94%。参与调查的专家中医师20名,西医师16名,中西医结合医师10名。(1)高度共识:中成药治疗CKD具有一定的优势特色,可延缓CKD3~5期(非透析)患者慢性肾脏病肾功能进展,可改善临床症状及提高生活质量,延缓肾功能降低、控制尿蛋白和血尿;(2)中度共识:当CKD3~5期(非透析)患者有瘀血、湿热等兼证,或腰酸、气短乏力、贫血等症状时,可选择适当的中成药治疗;(3)低度共识:合并恶心呕吐、皮肤瘙痒等症状,及钙磷代谢紊乱等并发症时,可选择中成药治疗。结论:参与调查的中医、西医和中西医结合医师的共识,说明在我国中(成)药治疗CKD有是普遍认可的治疗方法,具有一定的疗效和特色,可用于临床实践。
Objective:To assess the advantages and the status of Chinese patent medicine in the treatment of chronic kidney disease(CKD)to provide information for identifying clinical questions of the clinical practice guideline for Chinese patent medicine in the treatment of chronic kidney disease.Methods:From April 3 to April 17,2018,a total of 49 member of Nephrology Chinese Medicine Development Research Committee of China Association of Traditional Chinese Medicine experts were surveyed with expert questionnaires.All survey respondents were appointed with high professional titles and has a work experience of more than 10 years in a third-class A level hospital.Results:Forty-nine questionnaires were delivered and 46 were regained(94%response rate),including 20 Chinese medicine doctors,16 Western medicine doctors,and 10 Integrated Chinese and Western medicines doctors.There was a high consensus on the advantage of Chinese patent medicine in the treatment of CKD.The first advantage is delaying the progression of renal function in patients with CKD stage G3~5(non-dialysis).The second advantage is improving clinical symptoms and improve quality of life.A high consensus was achieved in delaying renal function,controlling urinary protein and hematuria for patients with CKD stage G3~5(non-dialysis)by Chinese patent medicine treatment.When CKD stage G3~5(non-dialysis)patients have symptoms such as blood stasis,damp-heat,waist soreness,shortness of breath,anemia,etc.,the choice of Chinese patent medicine treatment achieved a moderate consensus.When there are situations such as nausea and vomiting,pruritus,and disorder of calcium phosphate metabolism,the choice of Chinese patent medicine treatment achieved only a low consensus.Conclusion:There is a unique curative effect and advantage in the treatment of CKD with Chinese patent medicine,and there is a need for clinical practice.
