摘要
目的探讨电针"水沟"穴改善脑梗死大鼠脑循环的可能作用机制。方法将雄性Wistar大鼠130只随机分为模型组(40只)、电针组(40只)、假手术组(40只)和空白组(10只)。模型组、电针组以线栓法复制大脑中动脉栓塞(MCAO)模型,假手术组不插入线栓,空白组除抓取固定外不做任何处理。模型制备后除空白组外各组再随机分为3、6、12、24h 4个时相,每个时相10只。电针组在MCAO后即刻电针"水沟"穴20min,空白组、假手术组、模型组均同样抓取固定但不做其他处理。各组分别在各时相分离和剪取梗死侧的大脑前动脉、中动脉和后动脉各8mm,应用RT-PCR法检测血管平滑肌中三磷酸肌醇钙通道(IP3-Ca)、三磷酸肌醇(IP3)受体的3种亚基三磷酸肌醇受体1(IP3R1)mRNA、三磷酸肌醇受体2(IP3R2)mRNA、三磷酸肌醇受体3(IP3R3)mRNA表达量。结果与空白组及假手术组比较,模型组大鼠IP3R1 mRNA 3、6、12、24h时表达量显著升高(P<0.05),IP3R2 mRNA 3、6、12h时表达量显著升高(P<0.05或P<0.01),IP3R3 mRNA 3、6h时表达量显著升高(P<0.05)。与模型组比较,电针组IP3R1 mRNA 3h,IP3R2 mRNA 3、6h,IP3R3 mRNA 3h时表达量显著下降(P<0.05),其他时间与模型组比较差异无统计学意义(P>0.05)。结论电针干预可明显抑制MCAO模型大鼠脑血管平滑肌中IP3-Ca通道3种亚基IP3R1 mRNA、IP3R2 mRNA、IP3R3 mRNA表达量的升高,这可能是电针抑制缺血后脑血管平滑肌痉挛、保持血管功能和状态的正常,从而增加脑梗死区周围血流灌注的作用机制之一。
Objective To explore the possible mechanism of electroacupuncture at"Shuigou(GV26)"acupoint to improve the cerebral circulation in rats with cerebral infarction.Methods A total of 130 male Wistar rats were randomly divided into a model group(N=40),an electroacupuncture group(n=40),a sham operation group(n=40)and a blank group(n=10).The middle cerebral artery embolism(MCAO)model was duplicated by thread embolization and was administered in model group and electroacupuncture group.The thread was not inserted in the sham operation group.Only grasping and fixation were performed in the blank group.After the model was prepared,rats in groups except for blank group were randomly divided into 3 h,6 h,12 h and 24 h phases,with 10 rats in each phase.Rats in the electroacupuncture group were immediately needled at"Shuigou(GV26)"acupoint after MCAO,and were stimulated for 20 min,while rats in the model group,sham operation group and blank group were all grasped and fixed without treatment.The anterior,middle and posterior cerebral arteries in the infarcted side were separated and cut for 8 mm at each time phase in each group respectively.The real-time reverse transcription polymerase chain reaction(RT-PCR)method was used to detect the mRNA expression of the three subunits of inositol triphosphate(IP3)receptors of IP3-calcium channel(IP3-Ca),which were IP3 receptor 1(IP3 R1),IP3 receptor 2(IP3 R2)and IP3 receptor 3(IP3 R3).Results Compared to blank group and sham operation group,the mRNA expression of IP3 R1 at 3 h,6 h,12 h,24 h(P<0.05);the mRNA expression of IP3 R2 at 3 h,6 h,12 h(P<0.05 or P<0.01);and the mRNA expression of IP3 R3 at 3 h,6 h(P<0.05)of model group significantly increased.Compared to model group,the mRNA expression of IP3 R1 at 3 h,IP3 R2 at 3 h and 6 h,and IP3 R3 at 3 h of electroacupuncture group significantly decreased(P<0.05),while there were no statistical differences between model group and electroacupuncture group at other measurement time(P>0.05).Conclusion Electroacupuncture can significantly inhibit the increase of mRNA expression of IP3 R1,IP3 R2 and IP3 R3 of IP3-Ca channel in the cerebral vascular smooth muscle of MCAO model rats.This may explain the mechanism by which electroacupuncture can inhibit the cerebral vascular smooth muscle spasm after ischemia,and maintain normal blood vessel function and status,thereby to increase the blood perfusion around cerebral infarction area.
