糖尿病早期肾损伤诊断中尿糖、尿微量白蛋白联合检验的准确率及效果分析 被引量:23

摘要 目的分析将尿糖、尿微量白蛋白联合检验应用于糖尿病早期肾损伤诊断中的效果及准确率。方法90例糖尿病患者,根据是否存在早期肾损伤分为对照组(不存在早期肾损伤)及实验组(存在早期肾损伤),各45例。比较两组患者的尿微量白蛋白与尿糖水平,实验组单独检验及联合检验的诊断准确率。结果对照组患者的尿微量白蛋白水平为(17.30±3.05)mg/L,尿糖水平为(2.07±0.74)mmol/L;实验组患者的尿微量白蛋白水平为(32.59±4.12)mg/L,尿糖水平为(4.02±0.62)mmol/L。实验组患者的尿微量白蛋白、尿糖水平均高于对照组,差异具有统计学意义(t=20.0091、13.5498,P<0.05)。联合检验的诊断准确率高于单独尿糖及单独尿微量白蛋白检验,差异具有统计学意义(χ^2=5.8743、13.1408,P<0.05);单独尿糖检验与单独尿微量白蛋白检验的诊断准确率比较,差异无统计学意义(χ^2=1.7457,P>0.05)。结论将尿糖、尿微量白蛋白联合检验应用于糖尿病早期肾损伤诊断中具有理想效果,其诊断准确率更高,能够为临床诊断提供数据支持,可帮助临床医生判断患者是否存在早期肾损伤及其严重程度,值得在临床广泛推广应用。 Objective To analyze the effect and accuracy of combined test of urine glucose and microalbumin in the diagnosis of early kidney injury in diabetes mellitus.Methods A total of 90 patients with diabetes mellitus were divided into control group(no early kidney damage)and experimental group(early kidney damage)according to whether there was early kidney damage,with 45 cases in each group.Comparison was made on urine glucose and microalbumin levels between the two groups,and the diagnostic accuracy of single test and combined test in the experimental group.Results The microalbumin and urine glucose of the control group were(17.30±3.05)mg/L and(2.07±0.74)mmol/L,which were(32.59±4.12)mg/L and(4.02±0.62)mmol/L of the experimental group.The urine glucose and microalbumin levels of the experimental group were all higher than those of the control group,and the difference was statistically significant(t=20.0091,13.5498,P<0.05).The diagnostic accuracy of combined test was higher than single urine glucose and microalbumin test,and the difference was statistically significant(χ^2=5.8743,13.1408;P<0.05).There was no statistically significant difference in diagnostic accuracy between single urine glucose and microalbumin test(χ^2=1.7457,P>0.05).Conclusion The combined test of urine glucose and urine microalbumin has an ideal effect in the diagnosis of early renal injury of diabetes mellitus,and its diagnostic accuracy is higher.It can provide data support for clinical diagnosis and help clinicians determine whether patients have early renal injury and its severity.It is worthy of widespread clinical application.
