摘要
目的探讨经皮微创与开放式椎弓根螺钉内固定术治疗胸腰椎段脊柱骨折患者的效果。方法选择苏州市独墅湖医院于2018年1月至2020年1月收治的胸腰椎段脊柱骨折患者46例作为研究对象,采用随机数表法分为两组,各23例。对照组采取开放式椎弓根螺钉内固定术治疗,观察组采取经皮微创椎弓根螺钉内固定术治疗,记录两组患者的切口长度、出血量及术后恢复指标,测定胸腰椎功能参数变化。结果观察组手术切口长度、术中失血量、术后引流量、疼痛缓解时间及住院时间均显著低于对照组(P<0.05)。治疗后两组患者伤椎前缘高度、后凸Cobb角、椎体矢状面指数均明显改善,且比较差异无统计学意义(P>0.05)。结论与开放式手术相比,胸腰椎段脊柱骨折治疗采取经皮微创椎弓根螺钉内固定术具有创伤小、出血量少、恢复快的优势,胸腰椎功能改善程度相近,术后康复良好,值得推广。
Objective To investigate the effect of percutaneous minimally invasive and open pedicle screw fixation in the treatment of thoracolumbar spine fractures.Methods A total of 46 patients with thoracolumbar vertebral fracture admitted to Dushu Lake Hospital from January 2018 to January 2020 were selected as study subjects,and were divided into two groups by random number table method,with 23 patients in each group.The control group was treated with open pedicle screw internal fixation,while the observation group was treated with percutaneous minimally invasive pedicle screw internal fixation.The incision length,blood loss and postoperative recovery indexes were recorded,and the changes of thoracolumbar functional parameters were measured.Results The length of incision,intraoperative blood loss,postoperative drainage,pain relief time and hospital stay in the observation group were significantly lower than those in the control group(P<0.05).After treatment,the height of the anterior edge of the injured vertebrae,the Cobb angle of kyphosis,and the sagittal index of the vertebral body were significantly improved in the two groups,and the comparison was not statistically significant(P>0.05).Conclusion Compared with open surgery,percutaneous minimally invasive pedicle screw internal fixation for the treatment of thoracolumbar spine fractures has the advantages of less trauma,less bleeding,and faster recovery.The thoracic and lumbar spine function is improved to a similar degree,and the postoperative recovery is good.It is worth promoting.
