摘要
目的探讨肠内营养联合双歧杆菌三联活菌对神经介入治疗后急性缺血性脑卒中患者二胺氧化酶(DAO)、内皮素(ET)及营养指标的影响。方法选取2016年10月至2019年10月于南阳市中心医院神经介入科行神经介入治疗的急性缺血性脑卒中患者102例为研究对象,通过随机数字表法分为试验组(n=51)和对照组(n=51)。对照组接受肠内营养治疗,试验组在对照组基础上接受双歧杆菌三联活菌治疗,两组均治疗14 d。比较两组治疗前后神经功能、自理能力、肠道屏障指标、营养情况及治疗过程中并发症发生情况。结果治疗后,两组美国国立卫生院卒中量表(NIHSS)评分较治疗前均明显降低,试验组低于对照组(P<0.05);简化巴氏指数量表(BI)评分较治疗前均明显升高,且试验组高于对照组(P<0.05)。治疗后,两组血清DAO、ET水平较治疗前均明显降低,且试验组低于对照组(P<0.05)。治疗后,两组血清白蛋白(Alb)、前白蛋白(PA)及总蛋白(TP)水平较治疗前均明显降低,但试验组高于对照组(P<0.05)。试验组治疗过程中并发症发生率低于对照组(P<0.05)。结论肠内营养联合双歧杆菌三联活菌可有效降低神经介入治疗后急性缺血性脑卒中患者血清DAO、ET水平,改善肠道屏障功能及营养指标,进而促进神经功能恢复,提高患者生活质量,且并未增加并发症发生情况。
Objective To investigate the effects of enteral nutrition combined with bifidobacterium triple viable bacteria on diamine oxidase(DAO),endothelin(ET)and nutritional indexes in patients with acute ischemic stroke after neural intervention therapy.Methods A total of 102 patients with acute ischemic stroke who underwent neural intervention therapy in the Department of Neural Intervention in Nanyang Central Hospital from October 2016 to October 2019 were selected as the research objects,and were divided into the experimental group(n=51)and the control group(n=51)by random number table method.The control group received enteral nutrition treatment,while the experimental group received bifidobacterium triple viable bacteria treatment on the basis of the control group.Both groups were treated for 14 d.Nerve function,self-care ability,intestinal barrier index,nutritional status before and after treatment,and complications during the treatment were compared between the two groups.Results After treatment,the National Institutes of Health Stroke Scale(NIHSS)score in both groups was significantly lower than that before treatment,and that in the experimental group was lower than that in the control group(P<0.05).Simplified Barthel index(BI)score increased significantly,and that in the experimental group was higher than that in the control group(P<0.05).After treatment,serum DAO and ET levels in both groups were significantly lower than those before treatment,and those in the experimental group was lower than those in the control group(P<0.05).After treatment,serum albumin(Alb),prealbumin(PA)and total protein(TP)levels of the two groups were significantly lower than those before treatment,but those in the experimental group were higher than those in the control group(P<0.05).The incidence of complications during treatment in the experimental group was lower than that in the control group(P<0.05).Conclusion Enteral nutrition combined with bifidobacterium triple viable bacteria can effectively reduce the serum DAO and ET levels in patients with acute ischemic stroke after neural intervention therapy,improve intestinal barrier function and nutritional indexes,thereby promoting the recovery of nerve function and improving the quality of life of patients without increasing complications.
