摘要
目的观察脊柱微调手法治疗神经根型颈椎病的临床疗效。方法将80例神经根型颈椎病患者按照随机数字表法分为2组,对照组40例予常规推拿手法治疗,治疗组40例予脊柱微调手法治疗。2组均治疗28 d后统计疗效,比较2组治疗前后疼痛视觉模拟评分(VAS)、颈椎功能障碍指数量表(NDI)评分及简明健康状况调查表(SF-36)评分变化情况,比较2组治疗前后颈椎曲度及颈椎活动度(包括前屈、后伸、左屈、右屈、左旋及右旋)变化情况,比较2组治疗前后血清肿瘤坏死因子α(TNF-α)、白细胞介素(IL)1β及IL-6水平变化情况。结果治疗组总有效率92.5%(37/40),对照组总有效率85.0%(34/40),治疗组疗效优于对照组(P<0.05)。2组治疗后与本组治疗前比较,疼痛VAS及NDI评分均降低(P<0.05),SF-36评分均升高(P<0.05),且治疗组治疗后疼痛VAS、NDI评分及SF-36评分改善均优于对照组(P<0.05)。2组治疗后与本组治疗前比较颈椎曲度及颈椎前屈、后伸、左屈、右屈、左旋、右旋活动度均明显增加(P<0.05),且治疗组治疗后各项指标改善均优于对照组(P<0.05)。2组治疗后与本组治疗前比较TNF-α、IL-1β及IL-6水平均明显降低(P<0.05),且治疗组治疗后各项指标水平均低于对照组(P<0.05)。结论脊柱微调手法治疗神经根型颈椎病临床疗效确切,可明显改善患者临床症状,缓解疼痛,改善颈椎曲度及活动度,改善颈椎功能,提高生活质量,其作用机制可能与降低炎症因子水平,减轻局部炎症反应有关。
Objective To observe the clinical effect of fine-tuning manipulation on cervical spondylotic radiculopathy.Methods A total of 80 patients were randomly number divided into two of treatment and control group,with 40 patients in each group.Spinal fine-tuning manipulation was used as a treatment group(n=40).Traditional manipulation therapy was used as the control group(n=40).Both groups were treated 28 days.The changes of visual analogue scale(VAS)score,cervical spine dysfunction index(NDI)score and SF-36 score before and after treatment were compared between the two groups.The cervical curvature and cervical activity(including flexion,extension,left flexion,right flexion,left and right rotation)before and after treatment were compared between the two groups.Changes of serum tumor necrosis factor-α(TNF-α),interleukin(IL)1βand IL-6 levels before and after treatment in two groups.Results The total effective rate of the treatment group and the control group was 92.5%(37/40),85.0%(34/40),and the curative effect of the treatment group was better than that of the control group(P<0.05).After treatment,the VAS score,NDI score and SF-36 score of the two groups were increased compared with those before treatment(P<0.05),and the improvement of each index in the treatment group was better than that in the control group(P<0.05).The cervical curvature and cervical flexion,extension,left flexion,right flexion,left rotation and right rotation activity were significantly increased(P<0.05),and the improvement of each index in the treatment group was better than that in the control group(P<0.05).The levels of TNF-α,IL-1β and IL-6 were significantly decreased(P<0.05),and the levels of each index in the treatment group were lower than those in the control group after treatment(P<0.05).Conclusion The clinical efficacy of spinal fine-tuning manipulation in the treatment of cervical spondylotic radiculopathy is definite.It can significantly improve the clinical symptoms of patients,relieve pain,improve cervical curvature and activity,improve cervical function,and improve quality of life.Its mechanism may be related to reducing the level of inflammatory factors and reducing local inflammatory response.
