摘要
目的了解广州市医疗机构戒烟服务开展和利用情况,为提高医疗机构戒烟服务水平提供依据。方法 2018年,采用多阶段分层随机抽样方法,在广州市11个区抽取49家医疗机构1 686名医务人员和患者开展问卷调查。结果男性医护人员吸烟率为20.0%,男性就诊者吸烟率为38.1%。75.5%的医疗机构开设了戒烟门诊。医生和护士遇到吸烟就诊者会建议其戒烟的比例分别为58.3%和64.8%,差异有统计学意义(χ2=4.008,P<0.05)。57.8%的医生认为医生劝诫吸烟就诊者对其戒烟有促进作用,护士为68.4%(χ2=13.277,P<0.05)。24.3%的医生知道如何对吸烟者进行戒烟干预,86%以上的医护人员认为有必要对医护人员进行戒烟技能培训。23.0%的男性吸烟就诊者表示曾收到医生的戒烟建议,70%以上的就诊者认为医生指导戒烟更专业、更有说服力,61.2%的吸烟者听说过戒烟门诊,其中10.4%的吸烟就诊者曾在戒烟门诊就诊。结论广州市医疗机构的戒烟服务情况不理想,需强化医护人员戒烟服务意识,提高医护人员的戒烟技能水平,加大对戒烟门诊的宣传。
Objective To understand the development and utilization of smoking cessation services in medical institutions in Guangzhou,so as to provide evidence for medical institutions to improve the level of smoking cessation services.Method A multi-stage stratified random sampling method was used to conduct a questionnaire survey among 1686 medical staff and patients from 49 medical institutions in 11 districts of Guangzhou in 2018.Results The smoking rate of male medical staff was 20.0%,and that of male patients was 38.1%.And 75.5%of the medical institutions established smoking cessation clinic.There were 58.3%of the doctors and 64.8%of the nurses recommended the patients who were smokers to quit smoking,the difference was statistically significant(χ~2=4.008,P<0.05).And 57.8%of the doctors believed that the doctors’advices on quitting smoking could help smokers to quit smoking,which was 68.4%of the nurses(χ~2=13.277,P<0.05).Only 24.3%of the doctors knew how to conduct intervention on smoking,more than 86%of the doctors and nurses thought it was necessary to train them on smoking cessation skills.Only 23.0%of male smokers said that they had been advised by the doctors to quit smoking.More than 70%of the patients thought that the guidance from the doctors on quitting smoking was more professional and convincing,and 61.2%of the smokers had heard of smoking cessation clinic,and 10.4%of them had visited the clinic.Conclusion The situation of smoking cessation services in medical institutions in Guangzhou is not satisfactory.It is necessary to strengthen the awareness of smoking cessation services of medical staff,improve the level of smoking cessation skills of medical staff,and increase the publicity of smoking cessation clinic.
