微创经皮钢板固定技术结合锁定钢板治疗胫骨干粉碎骨折

Minimally invasive percutaneous plate fixation combined with locking plate in the treatment of comminuted tibial shaft fracture

摘要 目的观察探讨微创经皮钢板固定技术(minimally invasive percutaneous plate osteosynthesis,MIPPO)结合锁定钢板治疗胫骨干粉碎骨折的临床疗效。方法回顾性选择本院收治的52例胫骨干粉碎骨折患者,入组时间2017年11月至2019年10月。根据治疗方式进行分组,设为对照组与研究组。其中,对照组26例实施传统切开复位内固定术;研究组26例实施微创经皮钢板固定技术结合锁定钢板术。对比观察两组患者的临床治疗效果。结果评价临床疗效,研究组优良率高于对照组(88.4%vs 69.2%),差异显著,有统计学意义(P<0.05);研究组术中出血量、骨折愈合时间显著优于对照组(102.85±24.58 vs 214.57±29.81;9.21±1.89 vs 14.35±2.17),两组比较,差异显著,有统计学意义(P<0.05);统计并发症,研究组发生率低于对照组(3.8%vs15.3%),比较显示,差异有统计学意义(P<0.05)。结论对于胫骨干粉碎骨折,采取微创经皮钢板固定技术结合锁定钢板术式进行治疗可取得比较好的疗效,有推广价值。 Objective To investigate the clinical effect of minimally invasive percutaneous plate osteosynthesis(MIPPO)combined with locking plate in the treatment of tibial shaft comminuted fracture.Methods 52 cases of comminuted fracture of tibial cadres were selected retrospectively from November 2017 to October 2019.According to the treatment,the patients were divided into control group and study group.Among them,26 cases in the control group were treated with traditional open reduction and internal fixation,and 26 cases in the study group were treated with minimally invasive percutaneous plate fixation combined with locking plate.The clinical therapeutic effects between two groups were compared.Results The excellent rate of the study group was higher than that of the control group(88.4%vs 69.2%),and the difference was significant(P<0.05).The amount of bleeding and the time of fracture healing in the study group were significantly better than those in the control group(102.85±24.58 vs 214.57±29.81,9.21±1.89 vs 14.35±2.17),and the difference between two groups was significant(P<0.05).The incidence of complications in the study group was lower than that in the control group(3.8%vs 15.3%),and the difference was statistically significant(P<0.05).Conclusions For the comminuted fracture of tibial shaft,minimally invasive percutaneous plate fixation combined with locking plate operation can achieve the better effect,which is worth popularizing.
