乌市某三甲医院放射工作人员甲状腺功能及形态现况分析 被引量:3

摘要 目的了解长期低剂量电离辐射对从事放射工作的医务人员甲状腺功能及形态的影响,为放射工作人员职业健康管理提供指导依据。方法收集2019年1月至3月在新疆医科大学第一附属医院健康管理中心健康体检部体检的314名在职放射工作人员职业健康检查的基本信息、甲功五项、甲状腺超声结果,并选取同时间段进行健康体检的在职非放射医务人员的348人作为对照组,进行对照分析。结果放射组与对照组的促血清甲状腺激素(TSH)、三碘甲状腺原氨酸(T3)、甲状腺素(T4)、抗甲状腺球蛋白抗体(An-TG)、抗甲状腺过氧化物酶抗体(An-TPO)值都在正常范围内,两组间比较无显著性差异(P>0.05);各个年龄段之间及各个年龄段与对照组之间T、T4值的比较在统计学上无显著性差异,不同工种之间、不同工龄之间TSH、T3、T4、An-TG、An-TPO值比较在统计学上亦无显著性差异(P>0.05);放射组甲状腺多发结节检出率高于对照组(P<0.05),甲状腺无结节检出率低于正常组(P<0.05),而放射组甲状腺单发结节、疑似恶性结节的检出率与正常组间无显著差异(P>0.05)。结论日常放射工作不足以影响医用放射工作人员的甲状腺功能,但长期低剂量的电离辐射可增加患甲状腺多发结节的风险;因此,在加强从事放射工作医务人员的整体防护的同时,需注重放射工作人员职业健康管理。 Objective To understand the effect of long-term low-dose ionizing radiation on thyroid function and morphology of medical staff engaged in radiation work,and to provide guidance for occupational health management of radiation workers.Methods The basic information,five items of thyroid function and thyroid ultrasound results of 314 on-the-job radiation workers who had physical examination in the health examination department of a health management center from January to March 2019 were collected,and 348 non radiation medical staff who had physical examination at the same time period were selected as the control group for comparative analysis.Results The levels of serum thyroid stimulating hormone(TSH),triiodothyronine(T3),thyroxine(T4),antithyroid globulin antibody(an-tg)and antithyroid peroxidase antibody(an-tpo)in the radiation group and the control group were in the normal range,and there was no significant difference between the two groups(P>0.05).There was no significant difference in TSH,T3,T4,an TG,an-tpo values between different types of work and different length of service(P>0.05);the detection rate of multiple thyroid nodules in the radiation group was higher than that of the control group(P<0.05),and the detection rate of thyroid nodules in the thyroid gland was lower than that in the normal group(P<0.05).There was no significant difference between the two groups(P>0.05).Conclusions Daily radiation work is not enough to affect the thyroid function of medical radiation workers,but long-term low-dose ionizing radiation can increase the risk of multiple thyroid nodules;therefore,in strengthening the overall protection of medical staff engaged in radiation work,it is necessary to pay attention to occupational health management of radiation workers.
