幽门螺旋杆菌、血清Ghrelin与青年急性脑梗死患者神经功能损伤相关性分析 被引量:2

Correlation Analysis of Helicobacter Pylori,Serum Ghrelin and Neurological Damage in Young Patients with Acute Cerebral Infarction

摘要 目的:探究幽门螺旋杆菌(Hp)、血清胃饥饿素(Ghrelin)同青年急性脑梗死患者神经功能损伤的相关性。方法:选择2017年1月—2020年1月于我院接受治疗的84例青年急性脑梗死患者为研究组,另选取同期于我院接受体格检查的50例健康个体为对照组,将研究组患者按照美国国立卫生研究院卒中量表(NIHSS)得分区分为轻度组(1~4分,20例)、中度组(5~20分,34例)、重度组(21~42分,30例),就研究组各组与对照组的Hp感染率、血清Ghrelin水平进行比较,对研究组患者实施随访,以患者临床结局区分为死亡组(12例)和存活组(72例),对比不同临床结局患者Hp感染率和血清Ghrelin水平,就Hp感染率、血清Ghrelin水平与NIHSS量表评分相关性进行分析,最后绘制ROC曲线,计算Hp感染和血清Ghrelin对神经功能损伤的诊断价值。结果:研究组Hp感染率高于对照组,血清Ghrelin水平低于对照组,死亡组Hp感染率高于存活组,血清Ghrelin水平低于存活组(P<0.05);随着病情的加重,脑梗死患者的Hp感染率逐渐升高,Ghrelin水平逐渐降低,组间差异明显(P<0.05);Hp感染率同NIHSS量表评分呈现明显的正相关联系(r=0.9297,P<0.001),血清Ghrelin水平与NIHSS评分呈现明显的负相关联系(r=-0.9072,P<0.001);Hp感染率对神经功能缺损诊断AUC为0.5983(P=0.300),血清Ghrelin水平对神经功能缺损诊断AUC为0.8188(P<0.05)。结论:青年急性脑梗死患者Hp感染率会明显高于正常个体,Ghrelin水平低于正常个体,且随着病情加重出现明显变化,可以考虑将Hp感染率以及Ghrelin水平作为青年脑梗死患者病情评估和预后评价指标。 Objective:To explore the correlation between Helicobacter pylori(Hp)and serum ghrelin(Ghrelin)and neurological impairment in young patients with acute cerebral infarction(ACI).Methods:84 young patients with acute cerebral infarction who received treatment in our hospital from January 2017 to January 2020 were selected as the study group,and 50 healthy individuals who received physical examination in our hospital during the same period were selected as the control group.According to NIHSS score,the patients were divided into mild group(1~4 points,20 cases),moderate group(5~20 points,34 cases)and severe group(21~42 points,30 cases).The Hp infection rate and serum Ghrelin level of the study group were compared with those of the control group.The patients in the study group were followed up and divided into death group(12 cases)and survival group(72 cases)according to the clinical outcome Hp infection rate and serum Ghrelin level were compared between patients with different clinical outcomes,and the correlation between Hp infection rate,serum Ghrelin level and NIHSS score was analyzed.Finally,ROC curve was drawn to calculate the diagnostic value of Hp infection and serum Ghrelin for nerve function injury.Results:The Hp infection rate of the study group was higher than that of the control group,the serum Ghrelin level was lower than that of the control group,the Hp infection rate of the death group was higher than that of the survival group,and the serum Ghrelin level was lower than that of the survival group(P<0.05);with the aggravation of the disease,the Hp infection rate of cerebral infarction patients gradually increased,and the Ghrelin level gradually decreased,the difference between the groups was significant(P<0.05);the Hp infection rate was significantly positive with NIHSS score,there was a significant negative correlation between serum Ghrelin level and NIHSS score(P<0.001);the AUC of Hp infection rate in diagnosis of neurological deficit was 0.5983(P=0.300);AUC of serum Ghrelin level in diagnosis of neurological deficit was 0.8188(P<0.05).Conclusion:Hp infection rate of young patients with acute cerebral infarction is significantly higher than that of normal individuals,the Ghrelin level is lower than that of normal individuals.Hp infection rate and Ghrelin level can be considered as the evaluation index of the disease condition and prognosis of young patients with cerebral infarction.
