摘要
目的分析宁夏隆德县参与胃镜筛查与未参与胃镜筛查的胃癌患者5年生存状况,为宁夏制定胃癌防控策略提供数据支持。方法以隆德县2011年1月—2014年7月登记的316例胃癌患者作为研究对象,根据是否主动参与胃镜筛查分为筛查组和非筛查组,其中筛查组26例,非筛查组290例。采用Kaplan-Meier法分析5年观察生存率,用Log-rank法比较两组的生存曲线。结果隆德县胃癌年平均发病率为40.9/10万,死亡率为27.7/10万;胃癌患者5年观察生存率为18.8%(95%CI:14.5%~23.1%),中位生存时间为2.1年(95%CI:1.9~2.3)。筛查组患者5年观察生存率为52.1%(95%CI:32.3%~71.9%);非筛查组5年观察生存率为16.0%(95%CI:11.7%~20.2%),中位生存时间为2.1年(95%CI:2.0~2.2)。筛查组5年观察生存率高于非筛查组(P<0.05)。结论接受胃镜筛查,实施胃癌早诊早治,可提高胃癌患者5年观察生存率和生存时间均值。
Objective To analyze the 5-year survival status of gastric cancer patients who participated in and did not participate in gastroscopy screening in Longde County,Ningxia,so as to provide data support for formulating prevention and control strategies of gastric cancer in Ningxia. Methods A total of 316 cases of gastric cancer patients registered in Longde County from January 2011 to July 2014 were selected as the research objects,and they were divided into screening group and non screening group according to whether they actively participated in gastroscopy screening,including 26 cases in screening group and 290 cases in non screening group. Kaplan-Meier method was used to analyze the 5-year survival rate. Log-rank method was used to compare the survival curves of two groups. Results The annual incidence rate of gastric cancer in Longde county was 40.9/10 million,the mortality rate was 27.7/10 million,the 5 year survival rate of gastric cancer patients was 18.8%(95%CI:14.5%-23.1%),and the median survival time was 2.1 years(95%CI:1.9-2.3).The 5-year observed survival rate was 52.1%(95% CI: 32.3%-71.9%) in the screening group and 16.0%(95%CI: 11.7%-20.2%)in the non screening group,and the median survival time was 2.1 years(95%CI:2.0-2.2). The 5-year survival rate of screening group was higher than that of non screening group(P<0. 05).Conclusion Gastroscopic screening were performed in appropriate population, which will significantly increased the 5-year observed survival rate and mean survival time.
