摘要
从伦理审查经验出发,回顾性调查2016年到2018年期间上海市某儿童专科医院儿童性发育异常伦理审查情况,整理分析伦理审查过程,了解伦理审查在儿童性发育异常诊疗管理模式中的作用。共纳入儿童性发育异常伦理审查病例51例,整理分析相应的伦理审查会议记录19份,访谈相关医疗专家和伦理专家。三年间,儿童性发育异常诊疗服务的伦理审查数量呈现逐年增长,诊疗受到染色体、内分泌、外生殖器发育、心理、社会性别等多因素影响,其中患儿性别选择的风险受益评估、患儿心理评估情况、患儿家长的意见等是伦理审查的主要关注点。得出在伦理审查介入下,可推动儿童性发育异常多学科诊疗管理模式的构建,规范儿童性发育异常技术诊疗流程,促进性发育异常患儿心理建设,提高患儿的社会适应性。
Based on the experience of ethical review,a retrospective survey was conducted on the ethical review of children with disorders of sex development in a children's Hospital in Shanghai from 2016 to 2018.The process of ethical review was analyzed,to understand the role of ethical review in the management model of diagnosis and treatment of children with disorders of sex development.A total of 51 cases of sexual dysplasia of children were included,19 records of the corresponding ethical review meeting were analyzed,and related medical experts and ethics experts were interviewed.In the three years,the number of ethical review on the diagnosis and treatment of children with disorders of sex development has been increasing year by year.The diagnosis and treatment were affected by many factors such as chromosome,endocrine,external genitalia development,mind and social gender.Among them,the risk benefit assessment of children's gender selection,the psychological assessment of children,and the opinions of parents were the main concerns of ethical review.Under the intervention of ethical review,it is concluded that promote the construction of multidisciplinary diagnosis and treatment management model of children with sexual dysplasia,standardize the technical diagnosis and treatment process of children with sexual dysplasia,promote the psychological construction of children with sexual dysplasia,and improve the social adaptability of children.
