摘要 目的研究超声联合CT在鉴别诊断甲状腺结节中的应用价值。方法回顾性分析2019年5月至2019年12月在我院进行超声及CT检查的甲状腺结节患者108例(135个结节)的临床资料,均经手术病理学检查确诊,采用Kappa一致性分析两种检查方法及其联合与病理检查结果的一致性,计算其诊断甲状腺恶性结节的符合率、敏感度、特异度、准确性。结果 108例甲状腺结节患者共检出135个结节,其中良性结节99个(占73.33%),恶性结节36个(占26.67%);单一超声检查与CT检查诊断符合率比较无统计学意义(P>0.05),联合检查的诊断符合率显著高于两者(P<0.05);联合检测与病理结果一致性最高,敏感度和准确性均明显高于CT或者超声检查,特异度仅高于超声检查(P<0.05)。结论超声联合CT应用于甲状腺结节的鉴别诊断中,可有效提高诊断符合率,且敏感度、特异度以及准确性均较高。 Objective To study the application value of ultrasound combined with CT in the differential diagnosis of thyroid nodules.Methods The clinical data of 108 patients with thyroid nodules(135 nodules) who completed ultrasound and CT examinations in the hospital between May and December 2019 were retrospectively analyzed.All patients were confirmed by surgical and pathological examinations.The consistencies of the two examination methods and their combination with pathological results were analyzed through Kappa consistency analysis.Their coincidence rates,sensitivities,specificities,and accuracy rates for the diagnosis of malignant thyroid nodules were calculated.Results A total of 135 nodules were detected in the 108 patients,of which 99 were benign(73.33%),and 36 were malignant(26.67%).There was no statistically significant difference in the coincidence rate of ultrasound or CT alone(P>0.05).The diagnostic coincidence rate of combined examination of the two was significantly higher than that of single diagnosis of each method(P<0.05).The consistency of the combined detection and pathological results was the highest,and the sensitivity,and accuracy rate were significantly higher than those of CT or ultrasound.The specificity was only higher than that of ultrasound(P<0.05).Conclusion The application of ultrasound combined with CT in the differential diagnosis of thyroid nodules can effectively improve the coincidence rate of diagnosis,and the sensitivity,specificity,and accuracy are high.
