摘要
目的研究鼻内镜下不同入路方式治疗鼻腔鼻窦内翻性乳头状瘤(NIP)的效果及安全性。方法回顾性分析2017年4月至2019年8月我院收治的79例NIP患者的临床资料,根据手术方法的不同将患者分为A组(n=50,鼻内镜手术基础上行泪前隐窝入路)和B组(n=29,鼻内镜手术下联合行Caldwell-Luc入路)。比较两组患者的治疗效果。结果A组的手术时间、住院时间均明显短于B组,差异具有统计学意义(P<0.05)。A组的VAS评分低于B组,睡眠时间长于B组,差异具有统计学意义(P<0.05)。A组的并发症总发生率明显低于B组,差异具有统计学意义(P<0.05);两组的恶变率和复发率比较,差异无统计学意义(P>0.05)。结论与Caldwell-Luc入路相比,鼻内镜联合泪前隐窝入路治疗NIP,有利于患者术后恢复,且可降低并发症总发生率和复发率,值得推荐。
Objective To study the effect and safety of different approaches under nasal endoscope in the treatment of nasal inverted papilloma(NIP).Methods The clinical datas of 79 patients with NIP admitted in our hospital from April 2017 to August 2019 were retrospectively analyzed.According to different surgical methods,the patients were divided into group A(n=50,anterior lacrimal crypt approach based on nasal endoscopic surgery)and group B(n=29,Caldwell-Luc approach under nasal endoscopic surgery).The therapeutic effects of the two groups were compared.Results The operation time and hospital stay of the group A were significantly shorter than those of the group B,and the differences were statistically significant(P<0.05).The VAS score of the group A was lower than that of the group B,and the sleep time was longer than that of the group B,and the differences were statistically significant(P<0.05).The total incidence of complications in the group A was significantly lower than that in the group B,and the difference was statistically significant(P<0.05);there were no significant differences in the malignant transformation rate and recurrence rate between the two groups(P>0.05).Conclusion Compared with Caldwell-Luc approach,nasal endoscopy combined with anterior lacrimal crypt approach in the treatment of NIP is conducive to postoperative recovery,and can reduce the total incidence of complications and recurrence rate,which is worthy of recommendation.
