摘要
目的:分析布比卡因复合吗啡在子痫前期剖宫产中的临床效果。方法:2018年7月-2019年6月收治行剖宫产的子痫前期产妇110例,随机分为两组,各55例。对照组采用盐酸罗哌卡因连续硬膜外阻滞麻醉;观察组采用小剂量布比卡因复合吗啡腰硬联合麻醉。比较两组血流动力学指标水平、镇痛效果、肌肉松弛程度及新生儿Apgar评分。结果:观察组麻醉5 min、手术即刻、术毕时心率(HR)及平均动脉压(MAP)水平较麻醉前无明显变化,差异无统计学意义(P>0.05);对照组手术即刻时HR水平较麻醉前明显升高,麻醉5 min、手术即刻及术毕时MAP水平均较麻醉前明显降低,差异有统计学意义(P<0.05)。观察组术中肌肉松弛度优于对照组,术毕4 h视觉模拟评分法(VAS)评分明显低于对照组,差异有统计学意义(P<0.05);两组新生儿1 min Apgar评分比较,差异无统计学意义(P>0.05)。结论:子痫前期剖宫产术中应用布比卡因复合吗啡行腰硬联合麻醉的效果显著。
Objective:To analyze the clinical effect of bupivacaine combined with morphine in cesarean section of preeclampsia.Methods:From July 2018 to June 2019,110 cases of preeclampsia women with cesarean section were enrolled,they were randomly divided into two groups with 55 cases in each group.The control group was anesthetized by continuous epidural block of ropivacaine hydrochloride.The observation group received combined spinal and epidural anesthesia with low-dose bupivacaine combined with morphine.Hemodynamic indexes,analgesic effect,muscle relaxation and Apgar scores were compared between the two groups.Results:The observation group had no significant changes in heart rate(HR)and mean arterial pressure(MAP)levels at 5 minutes of anesthesia,immediately after the operation and at the end of the operation,the differences were not statistically significant(P>0.05).The HR level of the control group was significantly higher immediately than before anesthesia,after 5 minutes of anesthesia,immediately and after the operation,the MAP level were significantly lower than before the anesthesia,the differences were statistically significant(P<0.05).The muscle relaxation during operation in the observation group was better than that in the control group,VAS scores of 4 h after operation was significantly lower than that of the control group,the differences were statistically significant(P<0.05).Apgar score of 1 min between the two groups was compared,the difference was not statistically significant(P>0.05).Conclusion:Bupivacaine combined with morphine for combined spinal and epidural anesthesia in cesarean section of preeclampsia has postoperative analgesic effect.
