摘要
目的探究腹腔镜和开腹手术治疗急性阑尾炎的临床疗效和安全性。方法选取2016年1月至2019年1月本院收治的80例急性阑尾炎患者作为研究对象,按随机数字表法分为对照组和观察组,各40例。对照组行开腹手术治疗,观察组行腹腔镜手术治疗。比较两组围术期相关指标、炎症因子水平及术后并发症发生率。结果观察组术中出血量少于对照组,手术时间、切口长度、肛门排气时间、住院时间均短于对照组,疼痛评分低于对照组(P<0.05)。术前,两组炎症因子水平比较差异无统计学意义;术后,两组炎症因子水平均较术前改善,且观察组优于对照组(P<0.05)。观察组术后并发症发生率明显低于对照组(P<0.05)。结论与开腹手术比较,腹腔镜手术在急性阑尾炎患者的治疗中有效性和安全性更高,临床价值显著。
Objective To explore the clinical efficacy and safety of laparoscopy and laparotomy in the treatment of acute appendicitis.Methods 80 patients with acute appendicitis admitted to our hospital from January 2016 to January 2019 were selected as reseach subjects.According to the random number table method,they were divided into control group and observation group,with 40 cases in each group.The control group underwent laparotomy while the observation group underwent laparoscopic surgery.The perioperative indexes,inflammatory factors and postoperative complications were compared between the two groups.Results The amount of intraoperative blood loss in the observation group was less than that in the control group,the operative time,incision length,anal exhaust time and hospital stay time were shorter than those in the control group,and the pain score was lower than that in the control group(P<0.05).Before surgery,there was no significant difference in the levels of inflammatory factors between the two groups;after surgery,inflammatory factors in both groups were improved compared with that before surgery,and the observation group was better than the control group(P<0.05).The incidence of postoperative complications in the observation group was significantly lower than that in the control group(P<0.05).Conclusion Compared with open surgery,laparoscopic surgery is more effective and safe in the treatment of acute appendicitis and has more clinical value.
