冠脉CTA数据源3D打印主动脉根部模型的准确性研究

摘要 目的:探讨基于冠脉CT血管造影(CTA)医学数字成像和通信(DICOM)数据源3D打印主动脉根部实体模型的准确性。方法:回顾性对比分析30例于本院行外科主动脉瓣置换术患者的术前冠脉CTA DICOM数据,其中20例三叶式主动脉瓣(TAV)、10例二叶式主动脉瓣(BAV),将数据导入Mimics、3-matic医学影像后处理软件,完成主动脉根部3D数字模型重建后输入3D打印机产出实体模型。对比TAV组与BAV组原始数据以及两组原始数据与实体模型间相关参数值的统计学差异,分析两组原始数据与实体模型的主动脉瓣环各径线值一致性;分析两组实体模型瓣环各径线值与置换瓣型号的相关性;对比分析两组原始数据与病理标本、实体模型与病理标本的瓣膜钙化程度的一致性。结果:①TAV组与BAV组原始数据主动脉瓣环各参数值差异有统计学意义(P<0.05);②30例CTA的DICOM数据均成功3D打印主动脉根部实体模型,且两组原始数据与实体模型的各参数测值无统计学意义(均P>0.05)、原始数据与实体模型的瓣环径线参数测值一致性良好;③两组实体模型的瓣环径均为面积派生直径,与置换瓣型号相关性最强;④实体模型与病理标本对瓣膜钙化等级评估Kappa值为0.82,两者一致性良好。结论:基于冠脉CTA DICOM数据源可以准确地3D打印主动脉根部实体模型。 Objective:To analyze the accuracy of 3 D printed the solid model of aortic root based on the coronary computed tomography angiography(CTA) medical digital imaging and communications(DICOM) data.Methods:Preoperative coronary CTA DICOM data of thirty patients,including 20 cases of tricuspid aortic valve(TAV) and 10 cases of bicuspid aortic valve(BAV),who underwent surgical aortic valve replacement were retrospectively analyzed.We imported data into the medical imaging post-processing software of Mimics and 3-matic,completed the 3 D digital model reconstruction of the aortic root,and input it to the 3 D printer to produce the solid model.The statistical differences between the raw data of TAV group and BAV group were compared,and the consistency between the raw data and the solid model of the aortic annular diameter values of the two groups were analyzed.The correlation between the diameter values of aortic annular and the prosthesis size were analyzed.The consistency between the raw data and pathological specimens,and between the solid model and pathological specimens were analyzed.Results:① The aortic annular diameter values between the two groups were statistically different(P<0.05).②All the thirty patients’ 3 D solid models of aortic root were printed according to the coronary CTA DICOM data,and there was no statistical significance in the measured values of each parameter between the raw data and the solid model(all P>-0.05),and the consistency of aortic annular diameters between the raw data and the solid model were good.③The area derived diameter of the aortic annular in the solid model had the strongest correlation with prosthesis size in both groups.④The Kappa value of aortic valve calcium grade assessed by solid model and pathological specimens was 0.82,which showed a good consistency.Conclusion:Based on coronary CTA DICOM data,the aortic root solid model could be constructed by three-dimensional printer accurately.
