摘要
目的探讨在治艾滋病病毒(HIV)感染者/艾滋病(AIDS)病人(简称HIV/AIDS病人)CD4^(+)T淋巴细胞(简称CD4细胞)/CD8+T淋巴细胞(简称CD8细胞)(CD4/CD8比值)恢复的影响因素,为艾滋病抗病毒治疗(ART)提供理论依据。方法选择北京市房山区在治HIV/AIDS病人,将符合条件的病人按CD4/CD8比值分为恢复正常组(CD4/CD8比值≥1)和恢复偏低组(CD4/CD8比值<1),通过多因素Logistic回归分析探究影响CD4/CD8比值恢复的因素。结果共筛选出符合条件的HIV/AIDS病人379例,最近一次随访检测CD4/CD8比值≥1的病人68例(17.94%),<1的病人311例(82.06%)。单因素分析显示,性别、婚姻状况、治疗时长、疾病状态、基线CD4细胞计数、基线CD4/CD8比值是CD4/CD8比值恢复的影响因素。多因素Logistic回归分析结果显示,促进CD4/CD8比值恢复的因素有:疾病状态是HIV感染、较高的基线CD4细胞,阻碍CD4/CD8比值恢复的因素是较高的基线CD8细胞。结论北京市房山区大多数病例开始ART的时机较晚,早发现、早治疗是促进病人CD4/CD8比值恢复的关键。
Objective To explore the factors influencing the recovery of CD4/CD8 ratio of HIV/AIDS patients with ART(anti-virus treatment),and to provide a basis for ART.Methods According to the ratio of CD4/CD8,eligible HIV/AIDS patients in Fangshan district of Beijing were divided into normal group(CD4/CD8 ratio≥1)and low recovery group(CD4/CD8 ratio<1).Results Among a total of 379 eligible HIV/AIDS patients recruited,68 patients(17.94%)had CD4/CD8 ratio≥1 and 311(82.06%)had less than 1 CD4/CD8 ratio.Univariate analysis showed that gender,marital status,treatment duration,disease status,baseline CD4 cell count and baseline CD4/CD8 ratio were the influencing factors for the recovery of CD4/CD8 ratio.The results of multivariate logistic regression analysis showed that the factors promoting the recovery of CD4/CD8 ratio were early disease status of HIV,and the higher baseline of CD4 cells.The higher baseline of CD8 cells hindered the recovery of CD4/CD8 ratio.Conclusion Most cases in Fangshan district of Beijing started ART later.Early detection and treatment are key to promote the recovery of CD4/CD8 ratio.
