摘要
目的探讨痔疮自动套扎术(RPH)联合外痔切除缝合术对环状脱垂性混合痔患者创面愈合时间及并发症的影响。方法选取2018年4月~2020年4月廉江市人民医院肛肠科收治的64例环状脱垂性混合痔患者作为研究对象进行回顾性分析,根据不同的治疗方法将患者分为参照组(n=32)与研究组(n=32)。参照组患者采用传统内扎外剥术治疗,研究组采用RPH联合外痔切除缝合术治疗。观察和比较两组患者的治疗总有效率、创面愈合时间、住院时间、首次排便疼痛评分、肛门功能以及并发症总发生率。结果研究组患者的治疗总有效率高于参照组,差异有统计学意义(P<0.05)。研究组患者的并发症总发生率低于参照组,差异有统计学意义(P<0.05)。研究组患者的创面愈合时间、住院时间短于参照组,首次排便疼痛评分低于参照组,差异有统计学意义(P<0.05);研究组患者治疗后的肛管静息压、肛管舒张压高于参照组,差异有统计学意义(P<0.05)。结论对环状脱垂性混合痔患者进行RPH联合外痔切除缝合术治疗,效果显著,不仅能缩短创面愈合时间、住院时间,还能缓解首次排便的疼痛感,促进肛门功能的恢复,也能降低并发症发生率。
Objective To investigate the effect of automatic ligation of hemorrhoids(RPH)combined with external hemorrhoid excision and suture on wound healing time and complications in patients with annular prolapse mixed hemorrhoids.Methods A total of 64 patients with annular prolapse mixed hemorrhoids admitted to the Department of Anorectal in Lianjiang People's Hospital from April 2018 to April 2020 were selected as the research subjects for retrospective analysis,and the patients were divided into the reference group(n=32)and the research group(n=32)according to different treatment methods.The control group was treated with traditional internal ligation and external stripping,while the research group was treated with RPH combined with external hemorrhoid excision and suture.The total effective rate,wound healing time,hospitalization time,first defecation pain score,anal function and the total incidence of complications were observed and compared between the two groups.Results The total effective rate of the study group was higher than that of the reference group,and the difference was statistically significant(P<0.05).The total incidence of complications in the study group was lower than that in the reference group,and the difference was statistically significant(P<0.05).The wound healing time and hospitalization time of the study group were shorter than those of the control group,and the score of first defecation pain was lower than that of the control group,the differences were statistically significant(P<0.05).After treatment,the resting pressure and diastolic pressure of anal canal in the study group were higher than those in the reference group,and the differences were statistically significant(P<0.05).Conclusion RPH combined with external hemorrhoids resection and suture is effective in the treatment of annular prolapse mixed hemorrhoids.It can not only shorten the wound healing time and hospitalization time,but also relieve the pain of the first defecation,promote the recovery of anal function,and reduce the incidence of complications.
