摘要
目的探讨双歧杆菌三联活菌散联合非营养性吸吮对喂养不耐受早产儿血清胃肠激素水平的影响。方法选取2017年1月至2019年9月于台州医院住院治疗的76例喂养不耐受早产儿为研究对象,随机分为联合组和单用组各38例。单用组早产儿入院后即予以非营养性吸吮,联合组早产儿在单用组基础上加用双歧杆菌三联活菌散溶于母乳中口服或鼻饲给药,0.5 g/次,2次/d。两组对象连用3周。观察两组早产儿治疗前后血清胃肠激素水平的变化,并比较治疗后患儿恢复情况、治疗效果及不良反应。结果治疗3周后,两组早产儿血清胃动素(MTL)和胃泌素(GAS)水平较治疗前明显上升,且治疗后联合组上升幅度大于单用组(均P<0.05)。联合组早产儿呕吐消失时间、腹胀消失时间、恢复至出生体质量时间及达全胃肠喂养时间均明显短于单用组(均P<0.05)。联合组早产儿临床总有效率(94.7%)显著高于单用组(78.9%),差异有统计学意义(χ^(2)=4.15,P=0.0442)。单用组和联合组分别出现不良反应1例(2.63%)和3例(7.89%),症状均较轻,两组差异无统计学意义(χ^(2)=0.26,P>0.05)。结论双歧杆菌三联活菌散联合非营养性吸吮治疗喂养不耐受早产儿的效果较确切,安全性较高,能加快早产儿的恢复,其作用机制可能与其能促进MTL和GAS的分泌相关。
Objective To observe the effect of Triad Viable Bifidobacterium Powder combined with non-nutritive sucking on serum gastrointestinal hormone levels in preterm infants with feeding intolerance.Methods A total of 76 premature infants with feeding intolerance who were hospitalized in Taizhou Hospital from January 2017 to September 2019 were randomly divided into combination group or monotherapy group with 38 cases in each group.All the infants were given non-nutritive sucking after admission,while those in combination group were additionally given Triad Viable Bifidobacterium Powder dissolved in breast milk via oral or nasal administration,0.5 g bid,for 3 weeks.The changes of serum gastrointestinal hormone levels were observed,and the recovery,efficacy and adverse reactions were compared between groups.Results After 3 weeks of treatment,the levels of serum MTL and GAS in both groups significantly increased,with the combination group significantly superior to the monotherapy group(all P<0.05).The time to vomiting disappearance,time to distension disappearance,time to return to birth weight and time to total gastrointestinal feeding in combination group were significantly shorter than those in monotherapy group(all P<0.05).The total clinical efficiency rate in combination group was significantly higher than that in monotherapy group(94.7%vs 78.9%,χ^(2)=4.15,P=0.0442).One vs three cases of adverse readtions appeared in monotherapy group(2.63%)vs combination group(7.89%),with mild symptoms and no statistical differences between groups(χ^(2)=0.47,P>0.05).Conclusion Triad Viable Bifidobacterium Powder combined with non-nutritive sucking is effective for preterm infants with feeding intolerance,which has high safety and can accelerate the recovery of premature infants.Its mechanism of action may closely related to its potential to promote the secretion of MTL and GAS and increase serum levels of MTL and GAS.
