摘要
目的探讨手指末节动脉弓以远离断指体再植中"人"字形动脉吻合对再植成活及成活质量的影响。方法选取2016年1月-2018年12月收治的属YamanoⅠ型末节离断指中应用"人"字形动脉吻合再植指体68例74指,比较"人"字形动脉吻合端口径与叉口口径,对离断指体成活及成活质量的影响。结果YamanoⅠ型中,A组62指"人"字形动脉吻合口径明显大于叉口处管径,离断指极易出现再植体过度瘀紫,术后2~3d甚至需要临时换药处理,部分坏死2例,通过3周换药脱痂愈合,顺利成活;B组12指"人"字形动脉吻合口径等于或小于叉口处管径时,不易出现瘀紫,表现过度动脉充血,无需临时换药处理,完全成活。A组及B组术后持续随访时间24~54周,平均30.2周,观察再植指体外形饱满,质软,两点辨别觉3~7 mm,平均5 mm无感觉异常。结论"人"字形动脉吻合口径直接影响末节断指再植的成活及成活质量,更有促进评估断指再植预后情况。
Objective To investigate the effect of herring bone artery anastomosis of terminal arch of finger on survival and survival quality of replantation.Methods From january 2016 to December 2018,74 fingers in 68 cases of YamanoⅠtype of terminal digital amputated anastomotic were selected and were replanted with herring bone artery anastomosis.The survival and survival quality was compared,the caliber of herring bone artery and prong artery was also compared.Results The diameter of the anastomosis in group A was larger than that of the prong artery,it was very easy to show excessive stasis of the replant,and 2 to 3 days after operation,some cases of necrosis were treated by 3 weeks of dressing and scab removal.The diameter of anastomosis in group B was thiner the prong artery and had no above complications.The follow-up time of group A and group B was 24 to 54 weeks and the average followup time was 30.2 weeks.soft quality was obtained,two-point discriminant sense was S5,no sensory abnormalities occured.Conclusion Herring bone arterial caliber anastomosis directly affects the survival and survival quality of replantation of terminal amputated finger.
