摘要
目的分析彩色多普勒超声对甲状腺功能正常的桥本氏甲状腺炎(HT)患者临床诊断价值。方法选取商丘市第一人民医院2018年9月至2020年2月收治的76例甲状腺功能正常HT患者(研究组)、76例甲状腺功能亢进HT患者(对照1组)、76例甲状腺功能减退HT患者(对照2组)、76例健康人群(对照3组)作为研究对象。各组均行彩色多普勒超声检查,分别记录其甲状腺侧叶厚度、峡部厚度及内部血流情况。结果研究组甲状腺侧叶厚度均低于对照1组及对照2组,但研究组上述指标高于对照3组(P<0.05)。与其他各组比较,对照3组甲状腺峡部厚度最低(P<0.05),研究组、对照1组、对照2组甲状腺峡部厚度比较差异未见统计学意义(P>0.05)。研究组甲状腺动脉收缩期最大流速(PSV)、舒张末期血流速度(EDV)均低于对照1组及对照2组,但研究组上述指标高于对照3组(P<0.05)。各组阻力指数(RI)比较,差异未见统计学意义(P>0.05)。结论彩色多普勒超声检查对诊断甲状腺功能正常的桥本氏甲状腺炎病情具有重要的临床价值。
Objective To analyze the value of color Doppler ultrasound in the diagnosis of hashimoto’s thyroiditis(HT)with normal thyroid function.Methods Seventy-six patients with normal thyroid function HT(study group),76 patients with hyperthyroidism HT(control group 1),76 patients with hypothyroidism HT(control group 2)and 76 healthy people(control group 3)admitted to the First People’s Hospital of Shangqiu City from September 2018 to February 2020 were selected as study subjects.Color Doppler ultrasonography was performed in each group,and the thickness of lateral thyroid lobe,isthmus thickness and internal blood flow were recorded.Results The lateral thyroid thickness of the study group was lower than that of control group 1 and control group 2,but the above indicators in study group were higher than that of control group 3(P<0.05).The isthmic thyroid thickness in control group 3 was the lowest,compared with other groups(P<0.05).There was no significant difference in the isthmic thyroid thickness between the study group,control group 1 and control group 2(P>0.05).In the study group,the maximum systolic velocity(PSV)and thyroid artery end-diastolic blood flow velocity(EDV)were both lower than those in control group 1 and control group 2,but the above indicators in study group were higher than those in control group 3(P<0.05).The resistance index(RI)of each group was compared,with no significant difference(P>0.05).Conclusions Color Doppler ultrasonography has important clinical value in the diagnosis of hashimoto’s thyroiditis with normal thyroid function.
