摘要
目的探讨股骨髁间凹、胫骨髁间嵴、胫骨后倾角等解剖因素与前交叉韧带(ACL)损伤的关系。方法回顾性分析48例ACL损伤患者(ACL损伤组)和42例无ACL损伤的关节疼痛患者(对照组)的影像学资料。计算股骨髁间凹指数;通过正侧位X线片测量两组患者的内外侧胫骨髁间嵴高度和胫骨髁间嵴宽度,并通过MRI图像测量两组患者的股骨髁间凹宽度、股骨内外侧髁宽度以及胫骨内外侧平台后倾角。结果ACL损伤组胫骨髁间嵴宽度、胫骨髁间嵴宽度与股骨髁间凹宽度的比值均低于对照组,胫骨外侧平台后倾角大于对照组(均P<0.05),而两组胫骨内外侧髁间嵴高度、胫骨内侧平台后倾角、股骨髁间凹宽度及指数的差异均无统计学意义(均P>0.05)。结论狭窄的胫骨髁间嵴、较大的胫骨外侧平台后倾角、较小的胫骨髁间嵴宽度与股骨髁间凹宽度的比值可能与ACL损伤的发生有关。
Objective To explore the relation of anatomical factors,such as femoral intercondylar fossa,tibial intercondylar ridge and tibial posterior slope angle,with anterior cruciate ligament(ACL)injury.Methods The image data of 48 ACL injury patients(ACL injury group)and 42 arthralgia patients without ACL injury(control group)were retrospectively analyzed.The femoral intercondylar fossa index was calculated;the height of medial and lateral intercondylar ridge and the width of tibial intercondylar ridge of patients were measured on lateral and AP radiographs in the two groups.Besides,the width of femoral intercondylar fossa,the width between medial and lateral femoral condyle,as well as medial and lateral tibial posterior slope angle of patients were measured on MRI images in the two groups.Results The ACL injury group exhibited decreased width of tibial intercondylar ridge,lower ratio of tibial intercondylar ridge width to femoral intercondylar fossa width,and larger lateral tibial posterior slope angle as compared with the control group(all P<0.05),but there was no statistically significant difference between the two groups in height of medial or lateral intercondylar ridge,medial tibial posterior slope angle,or femoral intercondylar fossa width/index(all P>0.05).Conclusion Narrow tibial intercondylar ridge,greater lateral tibial posterior slope angle,decreased ratio of tibial intercondylar ridge width to femoral intercondylar fossa width may correlate with the incidence of ACL injury.
