摘要
目的探讨在初诊2型糖尿病患者中血清25(OH)D水平与非酒精性脂肪性肝病的关系。方法选取457例初诊T2DM患者,根据肝脏彩色超声结果将研究人群分为2组:对照组为T2DM无NAFLD者223例,NAFLD组为T2DM合并NAFLD者234例,对入选者进行体格检查、肝脏彩色超声检查及血脂、血糖、胰岛素、血清25(OH) D水平等相关指标的测定。结果 NAFLD组患者体质量指数(BMI)、腰围、甘油三酯(TG)、游离脂肪酸(FFA)、谷丙转氨酶(ALT)、γ-谷氨酰转移酶(γGGT)、谷草转移酶(AST)、空腹胰岛素(FINS)和胰岛素抵抗指数(HOMA-IR)均高于对照组,而血清25(OH) D水平低于对照组,且差异均有统计学意义(P <0.05)。Person相关分析提示血清25(OH) D水平与年龄(r=-0.205,P=0.003)、BMI(r=-0.189,P=0.015)、腰围(r=-0.161,P=0.020)、TG(r=-0.192,P=0.033)、FFA(r=-0.172,P=0.031)和HOMA-IR(r=-0.285,P <0.001)呈负相关。Logistic回归分析提示,血清25(OH) D水平是NAFLD的保护因素。结论在初诊T2DM患者中,血清25(OH) D水平与NAFLD密切相关,胰岛素抵抗可能未参与其中。
Objective To explore the association between serum 25-hydroxyvitamin D level and non-alcoholic fatty liver disease( NAFLD) in newly diagnosed type 2 diabetes( T2 DM). Methods A total of 457 subjects with newly diagnosed T2 DM were divided into two groups by hepatic ultrasonography,patients without NAFLD were control group and patients with NAFLD were NAFLD group. Their clinical,lipids,liver function,insulin,and serum 25-hydroxyvitamin D level were measured.Results Compared with control group,body mass index( BMI),waist circumference,triglyceride( TG),free fatty acids( FFA),alanine aminotransferase( ALT),γ-glutamyltransferase( γGGT),glutamic oxaloacetylase( AST),fasting insulin( FINS) and insulin resistance index( HOMA-IR) were all higher,while serum 25-hydroxyvitamin D level was lower,with the differences statistically significant( P < 0. 05). Person relevant analysis showed that serum 25-hydroxyvitamin D level was negatively correlated with age( r =-0. 205,P = 0. 003),BMI( r =-0. 189,P = 0. 015),waist circumference( r =-0. 161,P = 0. 020),TG( r =-0. 192,P = 0. 033),FFA( r =-0. 172,P = 0. 031) and HOMA-IR( r =-0. 285,P < 0. 001).According to Logistic regression analysis, serum 25-hydroxyvitamin D level is the protective factor for NAFLD.Conclusion In the early diagnosis of T2 DM,serum 25-hydroxyvitamin D level is significantly correlated with NAFLD and insulin resistance may not participate in this process.
