摘要
目的探讨不同程度类风湿性关节炎(RA)患者血清中肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、微小RNA-155(miR-155)、前白蛋白(PA)及转化生子因子-β1(TGF-β1)表达水平及其临床意义。方法选择2017年3月-2019年4月深圳市宝安区沙井人民医院风湿科门诊或住院部就诊并确诊为RA患者129例为RA组,同期来医院体检的无RA疾病的健康人群100名为对照组。采用酶联免疫吸附法(ELISA)检测血清中TNF-α、IL-6及TGF-β1水平,采用实时荧光定量PCR法检测外周血单核细胞(PBMC)中miR-155的相对表达水平,同时采用免疫比浊法检测PA水平,并对检测结果进行统计分析。结果 RA组血清中TNF-α、IL-6及PBMC中的miR-155水平分别为(41.73±12.05)pg/mL、(19.27±10.83)ng/mL及(1.82±0.53),明显高于对照组(19.86±6.57)pg/mL、(3.64±1.79)ng/mL及(1.20±0.47),而PA和TGF-β1水平分别为(135.62±27.63)mg/L和(258.49±27.16)pg/mL,明显低于对照组(278.94±43.05)mg/L和(457.26±80.37)pg/mL,差异均有统计学意义(P均<0.05)。血清中TNF-α、IL-6及PBMC中的miR-155水平高度活动组>中度活动组>轻度活动组,差异均有统计学意义(P<0.05),而PA和TGF-β1水平低度活动组>中度活动组>高度活动组,差异均有统计学意义(P<0.05)。经Spearman相关分析,TNF-α、IL-6及miR-155水平与RA患者病情活动程度呈正相关(r=0.761 4、0.697 2、0.605 4,P均<0.05),而PA和TGF-β1水平与RA患者病情活动程度呈负相关(r=-0.680 1、-0.592 4,P均<0.05)。结论 RA患者TNF-α、IL-6及miR-155水平明显升高,与病情活动度呈正相关,而PA和TGF-β1水平明显下降,与病情活动度呈负相关。
Objective To investigate the expression levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),micro RNA 155(miR-155),prealbumin(PA) and transforming growth factor-β1(TGF-β1) in the serum of patients with rheumatoid arthritis(RA) and explore their clinical significance.Methods From March 2017 to April 2019,129 RA patients who came to the rheumatic clinic or inpatient department of the hospital and were diagnosed as RA group;at the same time 100 healthy people who came to the hospital for physical examination were selected as control group.The levels of TNF-α,IL-6 and TGF-β1 were determined by enzyme-linked immunosorbent(ELISA);the relative expression level of miR-155 in peripheral blood mononuclear cell(PBMC.) was determined by real-time fluorescent quantitative PCR method;the level of PA in serum was determined by immune turbidimetric method,and the testing results were analyzed.Results The levels of TNF-α,IL-6 in serum and miR-155 in PBMC in RA group were(41.73±12.05)pg/mL,(19.27±10.83) ng/mL and(1.82±0.53),respectively,which were significantly higher than(19.86±6.57) pg/mL,(3.64±1.79) ng/mL and(1.20 ±0.47) in control group,while the levels of PA and TGF-β1 were(135.62±27.63) mg/L and(258.49±2.7.16) pg/mL,respectively,which were significantly lower than(278.94±43.05) mg/L and(457.26±80.37) pg/mL in control group,with statistically significant differences(P<0.05).The levels of TNF-α,IL-6 in serum and miR-155 in PBMC were high activity group>medium activity group>low activity group;the differenced were statistically significant(P<0.05),while the levels of PA and TGF-β1 in serum were low activity group>moderate activity group>high activity group;the difference was statistically significant(P<0.05).Spearman correlation analysis showed that the levels of TNF-α,IL-6 and miR-155 were positively correlated with the disease activity of RA patients(r=0.761 4,0.697 2,0.605 4,P<0.05),while PA and TGF-β1 were negatively correlated(r=-0.680 1,-0.592 4,P<0.05).Conclusion The levels of TNF-α,IL-6 and miR-155 in RA patients were significantly increased and positively correlated with disease activity,while the levels of PA and TGF-β1 were significantly decreased and negatively correlated with disease activity.
