摘要
背景颅骨修补术是神经外科的常见手术,但术后并发症仍是不可忽视的问题。目的探讨颅骨修补术中消融电极的应用效果。方法回顾性选取2012年1月—2020年1月航空总医院收治的行颅骨修补术的患者98例为研究对象。按术中分离及止血方式,将患者分为消融电极组45例及手术刀组53例。比较两组一般资料;观察并记录两组术后并发症(皮下积液、感染、癫痫、颅内血肿)发生情况及手术成功率。结果消融电极组术中出血量少于手术刀组(P<0.05)。消融电极组术后皮下积液发生率、总并发症发生率低于手术刀组(P<0.05);两组术后感染、癫痫、颅内血肿发生率及手术成功率比较,差异无统计学意义(P>0.05)。结论颅骨修补术中应用消融电极可明显减少术中出血量及降低术后并发症发生率,尤其在降低术后皮下积液发生率方面有明显优势,且手术成功率高,值得临床推广。
Background Cranioplasty is a common operation in neurosurgery,but postoperative complications are still a non-negligible problem.Objective To investigate the effect of ablation electrode in cranioplasty.Methods A total of 98 patients who underwent cranioplasty in Aviation General Hospital from January 2012 to January 2020 were retrospectively selected.According to the intraoperative separation and hemostasis methods,the patients were divided into ablation electrode group(n=45)and scalpel group(n=53).The general data of the two groups were compared;the incidence of postoperative complications(subcutaneous hydrops,infection,epilepsy,intracranial hematoma)and the success rate of surgery in the two groups were observed and recorded.Results The intraoperative blood loss in the ablation electrode group was less than that in the scalpel group(P<0.05).The incidence rate of postoperative subcutaneous hydrops and overall complications in ablation electrode group was lower than that in scalpel group(P<0.05).There was no significant difference in the incidence of postoperative infection,epilepsy,intracranial hematoma and the success rate of surgery between the two groups(P>0.05).Conclusion The application of ablation electrode in cranioplasty can significantly reduce the intraoperative blood loss and incidence rate of postoperative complications,especially in reducing the incidence rate of postoperative subcutaneous hydrops,with high success rate.It is worthy of being popularized in the clinical application.
