摘要
目的探讨胸腔镜肺癌根治术在肺癌患者中的应用及对代谢反应的影响。方法76例肺癌患者,用双色球分配法随机分为对照组和观察组,每组38例。对照组给予传统开胸肺癌根治术进行治疗,观察组实施胸腔镜肺癌根治术进行治疗。对比两组围手术期指标、手术前后代谢反应指标。结果两组淋巴结清扫量对比,差异无统计学意义(P>0.05);观察组手术时间(77.21±4.22)min、引流时间(3.11±0.54)d、住院时间(9.51±2.32)d均短于对照组的(94.31±5.25)min、(6.29±0.98)d、(18.29±3.26)d,术中出血量(142.68±15.28)ml少于对照组的(221.62±23.26)ml,切口长度(5.64±1.22)cm短于对照组的(29.54±3.21)cm,差异有统计学意义(P<0.05)。术后1周,观察组的前清蛋白(PA)、转铁蛋白(TRF)、视黄醇结合蛋白(RBP)水平高于对照组,胰高血糖素(GN)、血糖(GLU)、血清胰岛素(INS)水平低于对照组,差异均有统计学意义(P<0.05)。结论对肺癌患者应用胸腔镜肺癌根治术可减少对代谢反应的影响,有助于缩短手术时间,改善预后,值得推广。
Objective To discuss the application of thoracoscopic radical resection of lung cancer in patients with lung cancer and its influence on metabolic reactions.Methods A total of 76 patients with lung cancer were divided into control group and observation group according to two-color ball distribution method,with 38 cases in each group.The control group was treated with traditional thoracotomy for lung cancer,and the observation group was treated with thoracoscopic radical resection of lung cancer.The perioperative indexes and metabolic response indexes before and after the operation were compared between the two groups.Results There was no statistically significant difference in lymph node dissection between the two groups(P>0.05).The operation time(77.21±4.22)min,drainage time(3.11±0.54)d and hospitalization time(9.51±2.32)d of the observation group were shorter than(94.31±5.25)min,(6.29±0.98)d and(18.29±3.26)d of the control group,intraoperative blood loss(142.68±15.28)ml was less than(221.62±23.26)ml of the control group,and incision length(5.64±1.22)cm was shorter than(29.54±3.21)cm of the control group.The difference was statistically significant(P<0.05).1 week after operation,the levels of prealbumin(PA),transferrin(TRF)and retinol binding protein(RBP)were higher than those of the control group,while the levels of glucagon(GN),blood glucose(GLU)and serum insulin(INS)were lower than those of the control group,and the difference was statistically significant(P<0.05).Conclusion The application of thoracoscopic radical resection of lung cancer to lung cancer patients can reduce the impact on metabolic response,help shorten the operation time and improve the prognosis.It is worthy of promotion.
