摘要
腮腺炎性睾丸炎是流行性腮腺炎的常见并发症之一,其病因主要为感染腮腺炎病毒。疾病发生发展机制为:腮腺炎病毒从呼吸道进入人体进而复制侵入血液扩散至腮腺,之后增殖扩散至睾丸,引起睾丸的一系列病变。临床表现为腮腺肿大,开始消退时患者又出现发热,睾丸明显肿胀和疼痛。西医将其归为病毒性睾丸炎范畴,中医称为“痄腮引睾”,可见于儿童、青少年及成年男性,儿童未及时诊治会造成睾丸萎缩甚至导致成年后不育,成人感染腮腺炎后更易并发睾丸炎且临床症状较重。西医治疗此病主要通过常规补液支持治疗、抗病毒、抗感染治疗、补充雌激素及其他辅助疗法,中医辨证论治从清热解毒、疏肝泄热、清热养阴、疏肝理气散结、表里上下分消等方面入手,取得了显著疗效。近年,大量研究报道中西医结合相比单一疗法效果更佳,梳理近现代中西医治疗该病的个案、临床报道,为该病的诊疗提供参考。
Parotitis orchitis is one of the common complications of epidemic parotitis.Its main cause is infection with parotitis virus.The pathogenesis of the disease is as follows:the parotitis virus enters the human body from the respiratory tract,then replicates,invades the blood and spreads to the parotid gland,then proliferates and spreads to the testis,causing a series of lesions in the testis.Clinical manifestations include fever,testicular swelling and pain when parotid enlargement begins to subside.Western medicine classifies it as viral orchitis,which is called"parotitis induce orchitis"in traditional Chinese medicine(TCM),which can be seen in children,adolescents,and adult males.Failure to make timely treatment in children will cause testicular atrophy and even lead to infertility in adulthood.Adults infected with parotitis are more likely to be complicated with orchitis and severe clinical symptoms.Western medicine treats this disease mainly through fluid replacement support treatment,anti-virus treatment,anti-infection treatment,estrogen supplement,and other auxiliary therapy.TCM has achieved remarkable effects from the aspects of heat-clearing and detoxification,soothing the liver and relieving heat,clearing heat and nourishing Yin,soothing the liver and regulating Qi,dividing the exterior and interior,and so on.Recently,some studies have reported that the effect of integrated traditional Chinese and western medicine is better than single therapy.The cases and clinical reports of modern Chinese and western medicine in the treatment of the disease,are pulled together to provide a reference for the diagnosis and treatment of the disease.
