变应性鼻炎和支气管哮喘口鼻呼出气一氧化氮的相关性研究 被引量:9

Correlation between exhaled nitric oxide and nasal nitric oxide in allergic rhinitis and bronchial asthma

摘要 目的通过同时测定变应性鼻炎(AR)伴和不伴支气管哮喘(BA)患者的口呼出一氧化氮(FeNO)和鼻呼出一氧化氮(nNO),分析AR患者特应性状态与FeNO和nNO含量的相关性,评价其在AR和下气道炎症诊疗中的价值。方法选择诊断为持续性AR、BA、AR伴有BA(AR-BA)及对照组共4组各30例进入本研究。根据皮肤点刺试验(SPT)结果,将4组患者进一步分为高特应性组(SPT+++及以上)和一般特应性组(SPT++及以下)进行分析。所有受试者都接受了FeNO和nNO测量、变应原SPT、血绝对嗜酸性粒细胞计数和血清总IgE检测。结果3组试验对象的FeNO水平均高于对照组,差异有统计学意义(P<0.01)。AR-BA组FeNO水平高于BA组,差异有统计学意义(P<0.01)。AR组和AR-BA组nNO水平均高于对照组和BA组(P<0.01),但两者之间差异无统计学意义(P>0.05)。BA组nNO水平与对照组相比差异无统计学意义(P>0.01)。变应原的变态反应程度与FeNO及nNO呈正相关。结论FeNO和nNO可协助AR和哮喘的诊断,结果与患者的变态反应程度成正相关,nNO水平可作为BA并发AR的炎症标志物之一。FeNO亦可作为AR并发BA的炎症标志物,用于BA伴发哮喘的警示。 Objective To evaluate the correlation between fractional exhaled nitric oxide(FeNO)and nasal NO(nNO)in allergic rhinitis(AR)patients with or without bronchial asthma(BA).Methods In this study,patients suffering from persistent AR,BA,AR associated with BA(n=30 in each),and control(n=30)were included.According to the results of skin prick test(SPT),the participants were further divided into two groups:the tall atopic group(SPT+++and above)and the general atopic group(SPT++and below).All participants underwent the FeNO and nNO measurement,as well as the allergen SPT,the blood absolute eosinophil count,and the total serum IgE.Results The FeNO levels in the three groups were all significantly higher than those in the control group(P<0.01).The FeNO levels in the AR-BA group were significantly higher than those in the BA group(P<0.01).The levels of nNO in both the AR group and the AR-BA group were higher than those in the control group and the BA group(P<0.01)but there was no significant difference between the two groups(P>0.05).The level of nNO in the BA group was also significantly different from that in the control group(P<0.01).Conclusions FeNO and nNO are positively correlated with the degree of allergic rhinitis in patients with bronchial asthma,therefore,the patients’nNO levels can be used as one of the inflammatory markers of allergic rhinitis in patients with bronchial asthma.FeNO can also be used as an inflammatory marker of AR patients complicated with BA as a warning indicator of asthma in BA patients.
