摘要
目的:了解黑龙江省七台河市人民医院临床分离病原菌的分布情况及对抗菌药物的耐药性。方法:收集2019年1月1日-12月31日七台河市人民医院临床分离菌株,用自动化仪器法进行药敏试验,依据CLSI 2019版判读药敏结果,并采用WHONET 5.6软件进行数据分析。结果:2019年共收集临床分离菌3013株,其中革兰阳性菌632株(占21.0%),革兰阴性菌2381株(占79.0%);肺炎克雷伯菌、大肠埃希菌和金黄色葡萄球菌占检出病原菌的前3位;耐甲氧西林金黄色葡萄球菌(MRSA)占检出金黄色葡萄球菌的26.1%(59/226株),耐甲氧西林凝固酶阴性葡萄球菌(MRCNS)占检出凝固酶阴性葡萄球菌的63.9%(115/180株)。肠球菌对利奈唑胺和替考拉宁均表现为敏感。大肠埃希菌、克雷伯菌属和奇异变形杆菌中超广谱β-内酰胺酶的检出率分别为47.0%(209株)、15.3%(130株)、13.3%(8株)。肠杆菌科细菌对碳青霉烯类抗生素的耐药率较低,耐碳青霉烯类肠杆菌(CRE)的检出率为2.7%(44/1636株)。非发酵革兰阴性杆菌中,铜绿假单胞菌对亚胺培南和美罗培南的耐药率分别为23.4%(55/235株)和20.9%(49/235株),相比之下鲍曼不动杆菌对上述两种碳青霉烯类抗生素的耐药率较高,分别为68.7%(145/211株)和67.3%(142/211株)。结论:2019年我院MRSA检出率较低。发现1例屎肠球菌对万古霉素耐药。除万古霉素、利奈唑胺、替考拉宁对多种细菌敏感外,MRSA和MRCNS对利福平、肺炎链球菌对青霉素和左氧氟沙星,铜绿假单胞菌对哌拉西林/他唑巴坦、头孢他啶和阿米卡星的敏感率较高。
Objective:To investigate the distribution of pathogenic bacteria clinically isolated in Qitaihe People's Hospital in 2019 and their resistance to antibiotics.Methods:The clinically isolated strains were collected during Jan.1st-Dec.31st,2019,and drug sensitive test was performed with automated system.The results were analyzed with WHONET 5.6 software according to the 2019 breakpoints of Clinical and Laboratory Standards Institute.Results:A total of 3013 strains of pathogenic bacteria were isolated,of which 632(21.0%)were Gram-positive strains and 2381(79.0%)were Gram-negative strains.Klebsiella pneumonia,Escherichia coli,Staphylococcus aureus were the most common pathogens.About 26.1%(59/226 strains)of Staphylococsus aureus isolates and 63.9%(115/180 strains)of coagulase negative Staphylococcus(CNS)isolates were resistant to methicillin.No Enterococcus species were found resistant to linezolid or teicoplanin.The detection rate of extended spectrumβlactamases(ESBLs)was 47.0%(209 strains)in Escherichia coli,15.3%(130 strains)in Klebsiella sp.and 13.3%(8 strains)in Proteus mirabilis strains.Enterobacteriaceae strains showed a low resistance to carbapenem antibiotics,and the detection rate of carbapenemresistant Enterobacter sp.(CRE)was 2.7%(44/1636 strains).About 23.4%(55/235 strains)and 20.9%(49/235 strains)of Pseudomonas aeruginosa isolates were resistant to imipenem and meropenem.By contrast,the resistance rate of Acinetobacter baumannii to imipenem and meropenem was 68.7%(145/211 strains)and 67.3%(142/211 strains),respectively.Conclusions:The isolated rate of methicillin-resistant Staphylococcus aureus(MRSA)is lower in 2019 in authors’hospital.One strain of vancomycin-resistant enterococcus faecium is found.Vancomycin,linezolid and teicoplanin are still sensentive antibiotics for several kinds of bacteria.The sensitive rates of MRSA and methicillinresistant CNS(MRCNS)to rifampicin,Streptococcus pneumoniae to penicillin and levofloxacin,and Pseudomonas aeruginosa to piperacillin/tazobactam,ceftazidine and amikacin were highter.
