摘要
目的:探究血清胸苷激酶(thymidine kinase,TK1)、癌胚抗原(carcinoembryonic antigen,CEA)、糖类抗原(carbohydrate antigen,CA)72-4、CA242联合检测胃癌的诊断效能。方法:选择2018年2月至2019年2月我院收治的120例胃癌患者纳入胃癌组,另择100例同期因胃部不适于我院行胃镜检查的胃部疾病患者及50例查体的健康成年人分别纳入胃良性疾病组及对照组。测定受试者血清TK1、CEA、CA72-4、CA242水平,比较各组指标差异并绘制ROC曲线分析各指标的诊断效能。结果:胃癌组患者血清TK1、CEA、CA72-4、CA242显著高于胃良性疾病组及对照组患者(P<0.05);胃良性疾病组及对照组各指标比较差异无统计学意义(P>0.05);胃癌组患者中,进展期患者血清学指标显著高于早期胃癌患者(P<0.05);TK1、CEA、CA72-4和CA242单独诊断胃癌的曲线下面积分别为0.781、0.764、0.769、0.719;不同肿瘤标志物组织诊断胃癌的曲线下面积均大于单独诊断,其中四种联合及TK1+CEA+CA242三者联合诊断曲线下面积最大。结论:胃癌患者可伴有血清TK1、CEA、CA72-4和CA242的升高,各指标联合可提高胃癌的诊断效能。
Objective:To investigate the diagnostic efficiency of combined detection of serum thymidine kinase(TK1),carcinoembryonic antigen(CEA),carbohydrate antigen(CA)72-4 and CA242 for gastric cancer.Methods:120 patients with gastric cancer who were admitted to the hospital from February 2018 to February 2019 were included in the gastric cancer group.Another 100 patients with gastric diseases who underwent gastroscopy in the hospital for stomach discomfort and 50 healthy adults receiving physical examination during the same period were included in the benign gastric disease group and the control group.Levels of serum TK1,CEA,CA72-4 and CA242 were determined.The indicators were compared between the groups and the diagnostic efficiency of each index was analyzed by ROC curve.Results:The levels of serum TK1,CEA,CA72-4 and CA242 in the gastric cancer group were significantly higher than those in the benign gastric disease group and the control group(P<0.05),without significant difference between the benign gastric disease group and the control group(P>0.05).In the gastric cancer group,serum indexes in patients with advanced gastric cancer were significantly higher than those in patients with early gastric cancer(P<0.05).The areas under the curves of TK1,CEA,CA72-4 and CA242 alone for diagnosis of gastric cancer were 0.781,0.764,0.769 and 0.719,respectively.The areas under the curves of different tumor markers for diagnosis of gastric cancer were larger than that of single diagnosis.The area under the curve of combined diagnosis of the four indexes and TK1,CEA,CA242 combination was larger.Conclusion:The elevation of serum TK1,CEA,CA72-4 and CA242 is witnessed in patients with gastric cancer.The combined diagnosis of these indicators can improve the diagnostic efficiency for gastric cancer.
