阿替普酶在ACI治疗中对血清氧化应激血管内皮功能和脑神经功能的影响 被引量:9

摘要 目的:探讨阿替普酶在急性脑梗死(ACI)治疗中对血清氧化应激、血管内皮功能和脑神经功能的影响。方法:选取2018年9月至2020年9月我院160例ACI患者,根据抽签法进行分组,对照组(n=80)给予常规阿司匹林、氯吡格雷等治疗,观察组(n=80)在对照组治疗基础上予以阿替普酶治疗,比较两组治疗效果,治疗前后血清氧化应激、血管内皮功能和脑神经功能指标水平,以及不良反应。结果:观察组疗效总有效率95.00%高于对照组的83.75%(P<0.05)。治疗后,两组丙二醛(MDA)、内皮素(ET-1)水平和美国国立卫生院卒中量表(NIHSS)评分均下降(P<0.05),超氧化物歧化酶(SOD)、血管内皮生长因子(VEGF)水平和mRS评分均升高(P<0.05),且观察组治疗前后MDA、ET-1、SOD和VEGF水平差值及NIHSS、mRS评分差值均大于对照组(P<0.05)。两组不良反应发生率无显著差异(8.75%vs6.25%)(P>0.05)。结论:阿替普酶可以减轻ACI患者氧化应激反应和血管内皮损伤,改善脑神经功能。 Objective:To investigate the effect of alteplase on serum oxidative stress,vascular endothelial function and brain nerve function in patients with acute cerebral infarction(ACI).Methods:Between September 2018 and September 2020,160 patients with ACI were selected and randomly divided into 2 groups.The control group(n=80)underwent conventional treatment with aspirin and clopidogrel,while the observation group(n=80)underwent alteplase on the basis of the treatment for the control group.The 2 groups were compared with respect to treatment effects,serum oxidative stress,vascular endothelial function and cerebral nerve function indexes as well as adverse reactions.Results:The total response rate in the observation group was 95.00%,higher than 83.75%in the control group(P<0.05).After treatment,the levels of malondialdehyde(MDA)and endothelin(ET-1)and the National Institutes of Health Stroke Scale(NIHSS)were reduced,while the levels of superoxide dismutase(SOD)and vascular endothelial growth factor(VEGF)and mRS scores were increased in the two groups(P<0.05).In addition,the difference values of MDA,ET-1,SOD and VEGF levels,NIHSS and mRS scores before and after treatment were greater in the observation group than in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the 2 groups(8.75%vs 6.25%)(P>0.05).Conclusion:In sum,alteplase can reduce oxidative stress and vascular endothelial damage,and improve cerebral nerve function in patients with ACI.
