摘要
目的探讨CT图像后处理配合核磁共振(MRI)技术对踝关节骨折患者分型诊断与预后分析。方法选取龙岩市第二医院2015年4月—2018年4月收治的70例踝关节骨折患者,分别行CT及MRI检查,并运用CT图像后处理配合MRI技术对患者进行骨折位置、分型、分度的诊断,分析CT图像后处理配合MRI技术对踝关节骨折患者分型诊断及对预后的判断。结果CT图像后处理配合MRI检查显示,70例患者骨折位置中左踝37例,右踝33例;内踝29例,外踝33例,后踝8例。患者的骨折分型情况:SER 21例,SAD 14例,PER18例,PAB 17例,患者的骨折分度分布差异有统计学意义(P<0.05);CT图像后处理关节韧带显示情况:三角韧带71.42%(50/70),距腓前韧带54.28%(38/70),距腓后韧带51.42%(36/70),胫腓前韧带35.71%(25/70),胫腓后韧带42.85%(30/70);CT图像后处理配合MRI技术评估预后优秀、良好、尚可、差的占比与随访结果比较,差异无统计学意义(P>0.05)。56例手术患者中术后仅有3例发生创伤性关节炎,发生率为5.35%。结论踝关节骨折患者运用CT图像后处理配合MRI技术,可以精确地作出分型、分度诊断,对患者的预后有积极影响。
Objective To explore the application of CT image post-processing combined with magnetic resonance(MRI)in the diagnosis and prognosis of ankle fracture.Methods 70 patients with ankle fracture admitted to our hospital from April 2015 to April 2018 were selected and examined by CT and MRI respectively.The position,type and degree of fracture were diagnosed by CT image post-processing combined with MRI technology.The classification diagnosis and prognosis of patients with ankle fracture were analyzed by CT image post-processing combined with MRI technology.Results CT image post-processing combined with MRI showed that there were 37 cases of left ankle,33 cases of right ankle,29 cases of medial malleolus,33 cases of lateral malleolus and 8 cases of posterior malleolus.There were 21 cases of SER,4 cases of sad1,18 cases of per and 7 cases of PAB1.There were statistically significant differences in the distribution of fracture degree(P<0.05);the display of joint ligament after CT image post-processing:deltoid ligament 71.42%(50/70),anterior talofibular ligament 54.28%(38/70),posterior talofibular ligament 51.42%(36/70),anterior tibiofibular ligament 35.71%(25/70),posterior tibiofibular ligament 42.85%(30/70);CT image post-processing combined with MRI technology has excellent prognosis There was no significant difference in the proportion of good,fair and poor compared with the follow-up results(P>0.05).In 56 cases of surgical patients,only 3 cases of postoperative traumatic arthritis,the incidence rate was 5.35%.Conclusion In the classification diagnosis and prognosis analysis of patients with ankle fracture,the use of CT image post-processing combined with MRI technology can accurately make classification and classification diagnosis of ankle fracture,which has a positive impact on the prognosis of patients.
