摘要
目的了解某三甲医院2014—2018年医院感染现患率的分布特点,为优化院感防控措施提供合理化建议。方法采用横断面调查方法对某三甲医院2014—2018年同一调查日所有住院患者进行统一调查,对调查结果进行分析。结果2014—2018年全院应调查住院患者12358例,实查12252例,实查率为99.14%。共发生医院感染486例,医院感染现患率为3.97%;不同年龄组患者医院感染现患率差异有统计学意义(χ^(2)=33.908,P<0.001);感染部位主要以呼吸道、泌尿道感染和手术部位感染为主;医院感染主要集中在ICU、儿科、神经外科等院感防控重点科室;医院感染病原菌主要以革兰氏阴性菌为主(79.85%)。结论该医院感染现患率逐年降低,建议预防呼吸道、泌尿道医院感染的发生,加强重点科室防控工作,合理使用抗菌药物,从而做到科学、精准感控。
Objective To grasp the distribution characteristics of the current infection rate in a third-grade first-class hospital from 2014 to 2018,and provide reasonable suggestions for optimizing the hospital infection prevention and control measures.Methods The cross-sectional survey method was adopted to conduct a unified survey on all inpatients in a third-grade first-class hospital during the same survey day from 2014 to2018,and the survey results were analyzed.Results In 2014 to 2018 cross-sectional survey of nosocomial infection,12358 inpatients should be investigated,12252 cases were investigated,and the actual investigation rate was 99.14%.There were 486 cases of nosocomial infection,and the present infection rate was 3.97%.The prevalence rate of nosocomial infection in different age groups was statistically significant(χ^(2)=33.908,P<0.001).The main infection sites were respiratory tract and urinary tract infection and surgical site infection.Nosocomial infection mainly concentrated in ICU,pediatrics,neurosurgery and other hospital infection prevention and control key departments.The main pathogens of nosocomial infection were gram-negative bacteria(79.85%).Conclusion In order to prevent nosocomial infection in respiratory tract and urinary tract,we should strengthen the prevention and control work in key departments,rationally use antibiotics so as to achieve scientific and accurate infection control.
