摘要
目的:观察超声引导头部周围神经阻滞对颅骨修补手术患者术后镇痛的影响.方法:选择60例需行颅骨修补手术患者纳入本研究,随机分为2组,观察组(n=30)采取超声引导头部周围神经阻滞联合全身麻醉,对照组(n=30)采取全身麻醉,术后均给予静脉自控镇痛,比较术后不同时点MAP、HR变化,并统计术后不同时点视觉模拟评分(VAS)以评估术后疼痛情况,并统计术后不良反应发生情况.结果:观察组T1、T2、T3、T4时刻MAP、HR与T0时刻比较差异无统计学意义(P>0.05),但均低于对照组(均P<0.05);观察组T3、T4、T5时刻的VAS评分均低于对照组,且48小时内镇痛泵按压次数低于对照组(均P<0.05);观察组不良反应发生率低于对照组(P<0.05).结论:超声引导头部周围神经阻滞可减轻颅骨修补手术患者术后镇痛,血流动力学更为稳定,且不良反应少,具有重要临床价值.
Objective:To observe the effect of ultrasound-guided peripheral nerve block on postoperative analgesia in patients under-going cranioplasty.Methods:Select 60 patients needed cranioplasty patients include in this study,they were randomly divided into 2 groups,the observation group(n=30)took head peripheral nerve block guided by ultrasound in combination with general anesthesia,the control group(n=30)received general anesthesia,postoperative were given intravenous self-control analgesia,then to compare dif-ferent points of postoperative MAP,HR,and statistical different postoperative visual analogue scale(VAS)in the evaluation of postopera-tive pain,and statistics postoperative adverse reactions occur.Results:Compared with T 0 point,The difference of MAP,HR at T1,T2,T3 and T4 in the observation group was not statistically significant(P>0.05),but all were lower than the control group(P<0.05);VAS scores of the observation group at T3,T4 and T5 were all lower than those of the control group,and the number of analgesic pump com-pressions within 48h was lower than that of the control group(P<0.05);The incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).Conclusion:Ultrasound-guided peripheral nerve block can reduce postoperative analge-sia in patients undergoing cranioplasty and has more stable hemodynamics with fewer adverse reactions.
