摘要
目的:观察参芪益心方对慢性心力衰竭心气(阳)亏虚、血瘀水停型患者心功能、心肌能量消耗(MEE)及血清游离脂肪酸(FFA)等指标的影响,评价其临床疗效及安全性。方法:入组64例慢性心力衰竭心气(阳)亏虚,血瘀水停型患者,随机分为两组,剔除3例,对照组31例,治疗组30例。对照组予西医常规治疗,治疗组于此基础上联合参芪益心方,治疗3周,比较两组治疗前后中医证候积分、6分钟步行距离(6MWD)、左室射血分数(LVEF)、左心室收缩末圆周室壁应力(cESS)、MEE、血清NT-proBNP、血清FFA及安全性指标水平。结果:治疗后两组中医证候均有改善,治疗组总有效率优于对照组,差异具有统计学意义(P<0.05)。治疗后两组中医证候积分、cESS、MEE、血清NT-proBNP及FFA浓度较治疗前均降低,6MWD和LVEF均升高(P<0.05),治疗组均优于对照组,差异具有统计学意义(P<0.05)。结论:参芪益心方可以改善慢性心力衰竭心气(阳)亏虚、血瘀水停型患者的中医证候及心功能,同时可降低心衰患者MEE和血清FFA水平。
Objective:To observe the effect of Shenqiyixin prescription on heart function,MEE and serum FFA in patients with chronic heart failure of heart qi(yang)deficiency,blood stasis and fluid retention type,and to evaluate the clinical efficacy and safety.Methods:Totally 64 cases with chronic heart failure of heart qi(yang)deficiency with blood stasis and fluid retention were randomly divided into two groups,3 cases were excluded,31 cases in the control group and 30 cases in the treatment group.The control group was treated with western medicine,and the treatment group was treated with Shenqiyixin prescription additionally.The course of treatment was 3 weeks.The changes of TCM syndrome score,6MWD,LVEF,cESS,MEE,serum NT-proBNP,serum FFA and safety indexes of each group before and after treatment were observed.Results:After treatment,the TCM Syndromes of each group were improved.The total effective rate of the treatment group was 93.33%,and the control group was 83.87%.The treatment group was more effective(P<0.05).After treatment,the TCM syndrome score,cESS,MEE,NT-proBNP and FFA of each group were decreased,6MWD and LVEF were increased(P<0.05),and the treatment group was superior to the control group(P<0.05).Conclusion:Shenqiyixin prescription can improve the TCM syndrome and heart function in patients with chronic heart failure of heart qi(yang)deficiency with blood stasis and fluid retention,and at the same time,it can reduce the level of MEE and serum FFA in patients with heart failure.
