摘要
[目的]探讨血清高迁移率族蛋白1 (HMGB1)、转化生长因子-β1 (TGF-β1)在脊柱骨折脊髓损伤的参与机制和意义。[方法] 2017年1月~2019年6月,60例脊柱骨折伴脊髓损伤患者作为复合损伤组,60例单纯脊柱骨折患者作为单纯骨折组,50例健康人为健康对照组,采用ASIA评级评价复合损伤组脊髓功能,检测血清HMGB1和TGF-β1的水平。[结果] 1 d时,复合损伤组和单纯骨折组血清HMGB1和TGF-β1均明显高于健康对照组(P<0.05),复合损伤组在伤后1、3、7、14 d时,血清HMGB1和TGF-β1均明显高于单纯骨折组(P<0.05);随着ASIA分级的增加,复合损伤组血清HMGB1和TGF-β1的表达也明显更高(P<0.05);Pearson相关性分析显示,血清HMGB1和TGF-β1之间呈正相关(P<0.05)。[结论]血清HMGB1和TGF-β1在脊柱骨折伴脊髓损伤患者中明显比不伴有脊髓损伤的患者更高,随着脊髓损伤程度的不断加重,表达也逐渐升高,且血清HMGB1和TGF-β1之间呈明显正相关。
[Objective] To evaluated the significance of serum high mobility group protein 1(HMGB1) and transforming growth factor-β1(TGF-β1) in spinal fracture accompanied with spinal cord injury. [Methods] From January 2017 to June 2019, a total of 60 patients with spine fracture accompanied with spinal cord injury(the combined injury group), 60 patients with simple spinal fracture(the simple fracture group), and 50 health persons(the health control group) were enrolled in this study. The American Spinal Injury Association(ASIA) classification was used to evaluate neurological function in the combined injury group. The serum HMGB1 and TGF-β1 were assessed and compared among the 3 groups. [Results] At 1 day, the serum levels of HMGB1 and TGF-β1 in the combined injury group and the simple fracture group were significantly higher than the health control group(P<0.05). In addition, the serum HMGB1 and TGF-β1 in the combined injury group were significantly higher than the simple fracture group at 1, 3, 7 and 14 days(P<0.05). As ASIA classification upgrading, the serum levels of HMGB1 and TGF-β1 in the combined injury group significantly decreased(P<0.05). Pearson correlation analysis showed that there was a positive correlation between serum HMGB1 and TGF-β1(P<0.05). [Conclusion] The serum HMGB1 and TGF-β1 in the combined injury group is significantly higher than the simple fracture group. As spinal cord injury aggravating, the serum HMGB1 and TGF-β1 are escalating considerably. There is a positive correlation between serum HMGB1 and TGF-β1.
