摘要
经皮肾镜碎石取石术(PCNL)为上尿路结石的主要治疗方式之一,建立安全、高效的经皮肾通道是该技术的关键点。目前,PCNL相关技术和设备已较为完备,但由于肾脏血管及集合系统解剖的复杂性,出血、损伤、结石残留等手术并发症仍然存在。我们需要根据患者实际情况及手术条件,选择准确的穿刺路径并建立理想的操作通道。本文就肾脏血管及集合系统解剖研究与建立安全、高效PCNL通道之间的相关性及其在超声引导下经“肾盏穹隆-盏颈轴线”两步穿刺法中的应用进行分析探讨。
Percutaneous nephrolithotomy(PCNL)is one of the main treatments of upper urinary calculi,and the establishment of a safe and effective percutaneous renal channel is the key point of this technique.At present,the PCNL-related technique and equipment are relatively complete,but due to the complexity of renal vascular and collecting system,surgical complications still occur,such as bleeding,injury,and residual stones.We should know how to choose an accurate puncture route and establish a safe and effective PCNL channel according to the actual situation of the patients and surgical conditions.This article will analyze the correlation between the renal anatomy and establishment of an effective PCNL channel,and explore its application in ultrasound-guided two-step puncture through“calyceal dome-calyceal axis”.
