妊娠糖尿病孕妇肠道乳杆菌属和双歧杆菌属水平与糖脂代谢指标的相关性 被引量:5

Correlation between the levels of Lactobacillus and Bifidobacterium and the parameters of glucose and lipid metabolism in pregnant women with gestational diabetes mellitus

摘要 目的探讨妊娠糖尿病(GDM)孕妇肠道乳杆菌属和双歧杆菌属水平和糖脂代谢指标的相关性。方法选择2016年5月至2018年11月在宁波大学医学院附属医院常规体检并确诊为GDM的孕妇76例作为研究对象。将这76例孕妇设为GDM组。另选择同期在宁波大学医学院附属医院常规体检的76例非GDM孕妇作为对照组。比较两组的基线资料以及血糖、血脂、肠道乳杆菌属和双歧杆菌属水平,应用Pearson相关性分析乳杆菌属和双歧杆菌属与GDM组的血糖、血脂的相关性。结果两组的年龄、孕前体重指数(BMI)、孕周等基线资料比较,差异无统计学意义(P>0.05)。两组的血清空腹血糖(FPG)、OGTT 1h血清葡萄糖(1h-PG)、OGTT 2h血清葡萄糖(2h-PG)、总胆固醇(TC)、甘油三脂(TG)、高密度脂蛋白胆固醇(HDL-C)和低密度脂蛋白胆固醇(LDL-C)比较,差异具有统计学意义(P<0.05);GDM组的乳杆菌属和双歧杆菌属水平明显低于对照组,差异具有统计学意义(P<0.05)。GDM组肠道乳杆菌属水平和FPG、1h-PG、2h-PG、TC水平呈明显负相关(r=-0.535、-0.526、-0.634、-0.548,P=0.000),与HDL-C水平呈明显正相关(r=0.503,P=0.000);双歧杆菌属水平和FPG、1h-PG、2h-PG、TG、LDL-C水平呈明显负相关(r=-0.534、-0.540、-0.641、-0.621、-0.531,P=0.000)。结论GDM孕妇肠道乳杆菌属和双歧杆菌属的降低可能与其糖脂代谢紊乱存在一定的相关性,其具体机制需经进一步研究。 Objective To investigate the relationship between the levels of Lactobacillus and Bifidobacterium and the parameters of glucose and lipid metabolism in pregnant women with gestational diabetes mellitus(GDM).Methods 76 pregnant women diagnosed with GDM when received routine physical examination in the Affiliated Hospital of Medical School,Ningbo University from May 2016 to November 2018 were selected as the research subjects and assigned to the GDM group.76 non-GDM pregnant women who underwent routine physical examination in the Affiliated Hospital of Ningbo University were selected as the control group.Baseline data and blood glucose,blood lipid,intestinal Lactobacillus and Bifidobacterium levels of the two groups were compared,and Pearson correlation was used to analyze the correlation between Lactobacillus and Bifidobacterium and blood glucose and blood lipid of the GDM group.Results There was no significant difference between the two groups in age,pre-pregnancy body mass index(BMI),gestational age and other baseline data(P>0.05).The differences in FPG,1h-PG,2h-PG,total cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C)and low density lipoprotein cholesterol(LDL-C)between the two groups were statistically significant(P<0.05).The levels of Lactobacillus and Bifidobacterium in GDM group were significantly lower than those in control group,and the difference was statistically significant(P<0.05).The level of intestinal Lactobacillus in GDM group was significantly negatively correlated with the levels of FPG,1h-PG,2h-PG and TC(r=-0.535,-0.526,-0.634,-0.548,P=0.000),and positively correlated with the level of HDL-C(r=0.503,P=0.000).The level of bifidobacterium was negatively correlated with the levels of FPG,1h-PG,2h-PG,TG and LDL-C(r=-0.534,-0.540,-0.641,-0.621,-0.531,P=0.000).Conclusions The decrease of Lactobacillus and Bifidobacterium in pregnant women with GDM may be related to the disorder of glucose and lipid metabolism,and the specific mechanism needs further study.
