摘要
目的探究乳腺癌患者TOP2A基因变异的临床病理意义。方法选择2009年12月至2019年12月本院所收治的218例浸润性乳腺导管癌患者为研究对象。应用荧光原位杂交法测定乳腺癌样本内HER2基因和TOP2A基因变异详情。利用免疫组织化学法测定其他病理标志物HER2蛋白、Ki67、PR以及ER。分析结果。结果与TOP2A基因变异率相比,HER2基因变异率明显更高,HER2基因变异率和TOP2A基因变异率存在显著相关性(r=0.698,P<0.05),HER2蛋白变异和TOP2A基因变异存在相关性(r=0.3998,P<0.05),TOP2A基因变异和患者肿瘤直径、年龄无明显相关性,P>0.05,与组织学分级情况、淋巴结数量存在明显相关性,P<0.05。TOP2A基因于Luminal B型以及HER2过表达型呈现为高表达,其在Luminal A型以及基底细胞型为低表达。乳腺癌患者临床分子分型和TOP2A基因变异情况无相关性(r=-0.189,P>0.05)。结论乳腺癌患者的TOP2A基因变异和HER2变异存在相关性。可见,TOP2A基因变异代表患者预后不佳。
Objective To explore the clinicopathological significance of TOP2A gene variation in breast cancer patients.Methods A total of 218 patients with invasive ductal carcinoma of the breast admitted to our hospital from December 2009 to December 2019 were selected as subjects.Fluorescence in situ hybridization was used to determine HER2 and TOP2A gene variations in breast cancer samples.Other pathological markers,such as HER2 protein,Ki67,PR and ER,were determined by immunohistochemistry.Analyze the results.Results Compared with the TOP2A gene mutation rate,the HER2 gene mutation rate is significantly higher.There is a significant correlation between the HER2 gene mutation rate and the TOP2A gene mutation rate(r=0.698,P<0.05).There is a correlation between the HER2 protein mutation and the TOP2A gene mutation.(r=0.3998,P<0.05),there was no significant correlation between TOP2A gene mutation and the patient's tumor diameter and age,P>0.05,there was a significant correlation with histological grade and the number of lymph nodes,P<0.05.TOP2A gene is highly expressed in Luminal B type and HER2 overexpression type,and it is lowly expressed in Luminal A type and basal cell type.There was no correlation between clinical molecular typing of breast cancer patients and TOP2A gene mutation(r=-0.189,P>0.05).Conclusion There is correlation between TOP2A gene mutation and HER2 mutation in breast cancer patients.It can be seen that TOP2A gene variation represents a poor prognosis of patients.
