摘要
目的系统评价六味地黄丸联合西药对比单纯西药治疗高血压病的效果。方法通过检索万方、中国知网、维普、PubMed等数据库,检索时间从数据库建库至2019年3月,收集六味地黄丸治疗高血压病的随机对照试验(RCT)。根据Jadad量表评价标准对纳入文献进行资料提取和质量评价后,采用STATA 10.0统计软件进行Meta分析。结果最终纳入24篇文献,共计2457例患者,所纳入文献均为中文文献。Meta分析结果显示:六味地黄丸联合西药治疗高血压病的治疗组,与常规西药治疗的对照组相比,临床疗效:[RR=4.32,95%CI(1.26,7.39),P<0.05];降压疗效:[RR=2.73,95%CI(1.15,4.30),P<0.05];治疗后治疗组与对照组的血压值比较:收缩压[WMD=-4.33,95%CI(-5.76,-2.90),P<0.05],舒张压[WMD=-7.47,95%CI(-9.44,-5.49),P<0.05]。结论六味地黄丸联合西药治疗高血压病对比单纯西药能够更好降低收缩压与舒张压,在临床疗效与降压疗效上六味地黄丸联合西药更有优势。但是本次研究纳入的文献质量不高,未来仍然需要大量大样本、高质量的随机对照试验的理论支持。
Objective To systematically evaluate the efficacy of Liuwei Dihuang Pills(六味地黄丸)combined with Western medicine in the treatment of hypertension with western medicine alone.Methods By searching Wan Fang,CNKI,VIP,PubMed and other databases,the articles were searched from the establishment of databases to March 2019 to identify the randomized controlled trials(RCTs)of Liuwei Dihuang Pills in the treatment of hypertension.Meta-analysis was performed using STATA1.0 statistical software after data extraction and quality evaluation of the included literature according to the Jadad scale evaluation criteria.Results A total of 24 articles were included,totaling 2,457 patients,all of which were Chinese literature.Meta-analysis results showed that the effect of Liuwei Dihuang Pills combined with Western medicine treatment group for treating hypertension compared with the conventional western medicine control group was significant[RR=4.32,95%CI(1.26,7.39),P<0.05].The anti-hypertensive effect of the two groups were significant[RR=2.73,95%CI(1.15,4.30),P<0.05].Comparison of blood pressure values between the treatment group and the control group after treatment had significance[systolic pressure,WMD=-4.33,95%CI(-5.76,-2.90),P<0.05;diastolic pressure,WMD=-7.47,95%CI(-9.44,-5.49),P<0.05].The adverse events of Liuwei Dihuang Pills combined with conventional western medicine group treating hypertension were significantly less than those in the western medicine group.Conclusion Compared with western medicine,Liuwei Dihuang Pills combined with western medicine can better reduce systolic blood pressure and diastolic blood pressure,and Liuwei Dihuang Pills combined with Western medicine has advantages in clinical efficacy and anti-hypertensive effect.However,the quality of the literature included in this study is not high,and the theoretical support of a large number of large-scale,high quality randomized controlled trials is still needed in the future.
