摘要
目的研究控费政策对医院医疗业务收入和费用结构的影响,为完善新一轮控费政策提供依据。方法通过间断时间序列、结构变动度等方法,比较控费政策实施前后医院医疗业务收入和费用结构指标的变化。结果经模型检验,控费政策对医疗总收入、门诊次均费用、住院次均费用控制的即时效应显著,但尚未出现长期下降趋势;2018年下半年医院药占比下降7.64%,是医疗总收入下降的最主要因素;检验检查收入占比上升了3.22%,是医疗总收入下降的阻碍因素。结论控费政策初步控制了医疗费用的增长趋势,优化了医院收入结构,但需进一步强化政策的系统性与协调性,提高控费措施的精准性。
Objective To study the influence of fee control policy on hospital medical revenue and cost structure,and provide a basis for improving a new round of fee control policy.Methods The changes in hospital medical revenue and cost structure indicators before and after the implementation of the fee control policy were compared through using intermittent time series,structural change degree and other methods.Results After the model test,the fee control policy had significant immediate effects on the total medical income,the average outpatient cost,and the average hospitalization cost,but there is no long-term downward trend.The proportion of medicines in the second half of 2018 decreased by 7.64%,which was the most important factor in the decline in income.The proportion of medical examinations income rose by 3.22%,which was an obstacle to the decline in total medical income.Conclusion The fee control policy has initially controlled the growth trend of medical expenses and optimized the hospital’s income structure.However,it is necessary to further strengthen the systematicness and coordination of the policy and improve the accuracy of the cost control measures.
