摘要
目的:探讨术前贫血和贫血程度与胃癌并发症及预后的关系。方法:对2008年10月至2016年11月期间空军军医大学西京医院消化外科3692例行胃癌根治术患者的临床资料进行分析。探索术前贫血和贫血程度对胃癌术后并发症和预后的影响。结果:3692例患者中,男性2889例(78.3%),女性803例(21.7%)。贫血患者1493例(40.4%),年龄20~86岁,中位年龄58.5岁。术前贫血组年龄、性别、肿瘤部位、肿瘤大小、分化程度和T分期、N分期、TNM分期与术前未贫血组存在显著差异(P<0.05)。术前贫血组患者术后发热的发生率明显增高(P<0.05)。单因素分析结果显示,年龄、肿瘤大小、分化程度、T分期、N分期、TNM分期、术前贫血是影响胃癌预后的危险因素(P<0.05)。多因素分析结果显示,术前贫血是影响胃癌预后的独立危险因素(HR=1.141,95%CI:1.018~1.280,P=0.024)。在术前贫血组内,轻度、中度、重度贫血患者的生存无明显差异。结论:胃癌患者术前贫血会增加术后发热的发生率,术前贫血是影响胃癌预后的独立危险因素。
Objective:To analyze the association between pre-operative anemia and degree of anemia with the postoperative complications and prognosis of gastric cancer.Methods:From October 2008 to November 2016,3692 gastric cancer patients treated with radicalgastrectomy were enrolled in the present study.The association betweenpre-operative anemia and degree of anemia with postoperative complications and prognosis of gastric cancer and survival of patients were analysed.Results:There were 2889 males(78.3%)and 803 females(21.7%).The median age was 58.5 years(range 20~86 years old).The anemia group included 1493 patients(40.4%).Age,gender,tumor location,tumor size,differentiation status,T stage,N stage and TNM stage showed statistically significant differences between the anemia group and the non-anemia group(all P<0.05).The anemia group was associated with increased postoperative fever(P<0.05).Univariate analysis showed that age,tumor size,differentiation status,T stage,N stage,TNM stage and pre-operative anemia were risk factors for the prognosis of patients(all P<0.05).Multivariateanalysis showed that pre-operative anemia was independently associated with poor prognosis of patients(HR=1.141,95%CI:1.018~1.280,P=0.024).Conclusion:Pre-operative anemia was associated with postoperative fever.Pre-operative anemia was associated with poor prognosis of gastric cancer patients.
