摘要
目的:探讨左归丸、黄体酮联合小剂量二甲双胍治疗胰岛素抵抗型多囊卵巢综合征先兆流产患者的效果。方法:选取2018年10月-2020年5月本院临床妇产科治疗的胰岛素抵抗型多囊卵巢综合征伴先兆流产患者60例作为研究对象,随机分成试验组(n=30例)和对照组(n=30例),对照组采用小剂量二甲双胍联合黄体酮进行治疗,试验组在对照组基础上联合左归丸治疗。治疗后,对患者的临床治疗效果进行统计学分析,比较两组症状、血糖水平及不良妊娠结局。结果:治疗后,试验组安胎成功率高于对照组(P<0.05);流产率及胎死宫内率均低于对照组(P<0.05);治疗后,试验组空腹血糖、空腹胰岛素及胰岛素抵抗指数低于对照组(P<0.05);治疗后,试验组早产、胎盘早剥、产后出血及新生儿窒息等不良妊娠结局的发生率低于对照组(P<0.05)。结论:在临床治疗胰岛素抵抗型多囊卵巢综合征先兆流产患者中,采用左归丸、黄体酮联合小剂量二甲双胍进行治疗,能够提高安胎成功率,降低糖尿病发生率,改善血糖及胰岛素相关指标水平,降低不良妊娠结局。
Objective:To investigate the effect of Zuogui Pill,Progesterone combined with low-dose Metformin in the treatment of threatened abortion in patients with insulin resistance polycystic ovary syndrome.Method:From October 2018 to May 2020,60 patients with insulin resistant polycystic ovary syndrome with threatened abortion were selected as the research objects and randomly divided into the experimental group(n=30)and the control group(n=30).The control group was treated with low-dose Metformin combined with Progesterone,and the experimental group was treated with Zuogui Pill on the basis of the control group.After treatment,the clinical treatment effect of the patients was statistically analyzed,and the symptoms,blood glucose levels and adverse pregnancy outcomes of the two groups were compared.Result:After treatment,the success rate of abortion in the experimental group was higher than that in the control group(P<0.05);the abortion rate and intrauterine death rate were lower than those in the control group(P<0.05);after treatment,the fasting blood glucose,fasting insulin and insulin resistance index in the experimental group were lower than those in the control group(P<0.05);after treatment,the incidence of adverse pregnancy outcomes such as the preterm birth,placental abruption,postpartum hemorrhage and neonatal asphyxia occurred in the experimental group was lower than that of the control group(P<0.05).Conclusion:Zuogui Pill,Progesterone combined with low-dose Metformin in the treatment of threatened abortion in patients with insulin resistance polycystic ovary syndrome can improve the success rate of abortion,reduce the incidence of diabetes,improve the level of blood glucose and insulin related indicators,and reduce the adverse pregnancy outcome.
