摘要
目的:比较声触诊组织成像量化(VTIQ)技术、剪切波弹性成像(SWE)技术对乳腺肿块的诊断价值。方法:对135例患者(145个乳腺病灶)先行术前常规超声检查,并进行BI-RADS分类,后行VTIQ检查,获得剪切波速度平均值(SWVmean),同时行SWE检查,获得弹性模量最大值(Emax)、弹性模量平均值(Emean)、弹性标准差(Esd)及弹性比值(Eratio)。绘制BIRADS分类及各弹性参数诊断乳腺病灶良恶性的受试者工作特征(ROC)曲线。比较BI-RADS分类、VTIQ及SWE的曲线下面积(AUC)差异,并比较两种诊断方法在不同BI-RADS亚分类中的准确率差异。结果:AUC最大的SWE参数为Emax,VTIQ、SWE、BI-RADS分类的AUC分别为0.912、0.909、0.845。VTIQ与SWE的AUC比较差异无统计学意义,但两者与BI-RADS分类比较差异均有统计学意义(P<0.05)。VTIQ鉴别诊断BI-RADS 4A的准确率高于SWE,差异有统计学意义(P<0.05);对BIRADS 4B和4C类病灶,两者准确率比较差异均无统计学意义。结论:VTIQ与SWE对乳腺良恶性病灶的鉴别诊断均具有较高价值,VTIQ对BI-RADS 4A类病灶具有更好的诊断价值。
Objective:To compare the diagnostic value of virtual touch tissue imaging quantification(VTIQ)and shear-wave elastography(SWE)in breast masses.Methods:One hundred and thirty-five patients(145 breast lesions)were examined by routine ultrasound and classified by BI-RADS,followed by VTIQ examination to obtain the average shear wave velocity(SWVmean),and SWE examination to obtain the maximum elasticity(Emax),mean elasticity(Emean),standard deviation of elasticity(Esd)and elasticity ratio(Eratio).The receiver operating characteristic(ROC)curves of BI-RADS classification and elastic parameters in the diagnosis of benign and malignant breast lesions were drawn.The AUC differences of BI-RADS classification,VTIQ and SWE were compared,and accuracy differences of the two diagnostic methods in different BI-RADS subclassifications were compared.Results:The maximum SWE parameter of AUC was Emax,and the AUC of VTIQ,SWE and BIRADS were 0.912,0.909 and 0.845,respectively.There was no statistically significant difference in AUC between VTIQ and SWE,but there was statistically significant difference in AUC between VTIQ and SWE and BI-RADS(P<0.05).VTIQ was more accurate in differential diagnosis of BI-RADS 4A than SWE,and the difference was statistically significant(P<0.05).For BI-RADS 4B and 4C lesions,the difference in accuracy was not statistically significant.Conclusion:VTIQ and SWE are of high value in the differential diagnosis of benign and malignant breast lesions,and VTIQ is of better diagnostic value for BIRADS 4A lesions.
